Resident Care Manager (RCM)
Reports To: Administrator/Director of Nursing

Position Summary
The Resident Care Manager (RCM) is responsible for coordinating and overseeing all aspects of resident care to ensure compliance with state regulations (including OAR 411), company policies, and best practices in senior care. This role manages care plans and documentation, ensures timely communication with primary care providers (PCPs), and supports the clinical team in delivering high-quality, person-centered care.
Key Responsibilities
Daily Responsibilities
Bowel Movement Monitoring: Review Bowel Movement Logs daily to identify trends and potential constipation issues.
Missed Medication Oversight: Run and review the Missed Medication Report; follow up with med-techs to ensure documentation completion.
Pending Orders Management:
Review the Pending Orders Tab in PCC to confirm orders have been received.
Verify medications are in the building.
Confirm medication profiles are accurate.
Create or update Treatment Service Plans (TSPs) for new medications, including monitoring parameters (side effects, behaviors).
Skin Checks/Body Audits: Ensure weekly skin checks are completed and signed; assist LPNs as needed. Confirm second checks are performed by med-techs when new issues arise.
PCP Communications: Check 24-hour cottage-specific communication books for pending PCP communications. If unanswered after one week, resend to PCP.
Alert Charting Oversight:
Run and review the Alert Charting Report daily.
Ensure med-techs have completed all alert charting.
Create/update matching TSPs based on new alert charting entries (behaviors, symptoms, interventions).
Fax & Documentation Management: Fax responses from PCPs to Consonus for medication profile updates. Create/update TSPs for any new or changed medications following PCP response.
Incident Reports: Review incident reports with the clinical team. Develop individualized interventions with the nurse, add interventions to TSPs, and write root cause analyses. Review and update interventions for falls or other risk areas arising from incident reports.
Weekly Responsibilities
Medication Audits: Perform spot audits of med carts for correct labeling, storage, and expiration dates.
PRN Medication Oversight: Check PRN cabinets for expired medications.
90-Day Med Reviews: Follow up to ensure timely completion of 90-day medication reviews.
Service Plan Verification: Verify Service Plans in cottages are up to date and compliant with OAR 411 standards.
Service Planning (OAR 411)
Develop, review, and update individualized Service Plans in accordance with Oregon Administrative Rules (OAR 411).
Collaborate with residents, families, PCPs, and care staff to ensure plans reflect current needs, risks, and resident preferences.
Monitor implementation of Service Plans and ensure interventions are documented and followed consistently.
Qualifications
Knowledge of state regulations (including OAR 411 for assisted living and memory care).
Some experience as a Resident Care Manager (RCM) and/or Resident Care Coordinator (RCC) in an Assisted Living Facility (ALF) or Memory Care setting.
Strong organizational and documentation skills with attention to detail.
Excellent communication and problem-solving skills.
Physical & Working Conditions
Ability to lift up to 25 lbs. occasionally.
Work performed in a healthcare/residential environment with frequent resident interaction.

Pay: $22.00 - $24.00 per hour
