Job Description

Job Summary



The Resident Care Coordinator (RCC) is a licensed professional responsible for supervising personal care services and clinical operations for residents within the community. This position plays a vital leadership role in ensuring residents receive the highest quality of care, that all regulatory standards are maintained, and that the care team operates efficiently and compassionately. The RCC functions under the direction of the Director of Wellness and Executive Director, serving as a key liaison among residents, families, staff, and physicians.

Resident Care Oversight



Conduct daily room sweeps to ensure the community is free from restraints, rails, and unsafe chemicals.

Notify PCPs of abnormal vitals and document follow-up notes promptly.

Maintain accurate and up-to-date forms for admission, quarterly reviews, and changes in condition.

Complete service plan updates quarterly and as needed based on resident status.

Conduct care plan meetings with families/POAs and complete documentation in PCC.

Ensure assessments of prospective residents are completed within 24 hours of inquiry.

Maintain timely and accurate incident reports.

Clinical & Medication Management



Monitor Med Techs and Nurses to ensure all vitals and weights are entered and verified by the 5th of each month.

Conduct quarterly MedTech/LPN skills checkoffs and maintain competency documentation.

Oversee weekly medication cart audits, including pharmacy concerns, refill requests, and expired medications.

Ensure medication rooms are organized and compliant.

Ensure labs are collected and processed accurately and on schedule.

Staffing & Leadership



Serve as the first line of contact for staffing issues and schedule changes.

Manage weekday schedules and rotate on-call weekends with the Wellness Director.

Demonstrate flexibility and the ability to cover staffing call-outs when needed to ensure continuity of care

Oversee new staff orientation and training utilizing a 1-3 day onboarding checklist.

Supervise Med Techs and Resident Aides, enforcing standards and discipline when necessary.

Provide ongoing coaching, education, and competency assessments for care staff.

Resident Admissions & Transitions



Enter all new move-ins into PCC, including demographics, vitals, photos, and skin assessments.

Transmit pharmacy, therapy, and provider packets/orders.

Ensure all necessary safety signage (e.g., Oxygen, DNR) is in place.

Maintain proactive communication with families and POAs.

Oversee return-to-community transitions from hospital or rehab, completing new orders and skin assessments.

Quality Assurance & Reporting



Maintain and submit weekly reports including missed/late medication reports and barcode scanning.

Maintain shower, linen, and laundry schedules for all residents.

Update the Radar Report weekly and communicate changes to the Wellness Director.

Maintain Emergency Binders and ensure a two-week supply of essential medical items is on hand.

Scan and upload all relevant documentation into PCC timely and accurately.

Family & Physician Communication



Communicate promptly with families/POAs regarding any changes or concerns.

Notify the Wellness Director immediately of any changes in condition or care needs.

Coordinate with physicians, pharmacies, and therapy providers to ensure resident continuity of care.

Maintain professional email communication and respond promptly to correspondence.

Education and Experience



Licensed Practical Nurse (LPN) with a minimum of 2 years' experience in a long-term care or senior living setting.

Demonstrated experience supervising CNAs/Med Techs and managing care delivery systems.

Proven record of compliance with state regulations and internal policies.

Knowledge, Skills, and Abilities



Strong leadership and decision-making skills.

Excellent organization, documentation, and communication abilities.

Compassionate, resident-centered approach to care.

Ability to work weekends and respond to urgent needs as required.

Proficient in PCC and electronic medical records.

CPR and First Aid certified.

Physical Requirements



Ability to lift 50 lbs., stand, walk, and sit for extended periods.

Flexibility to adjust to changing schedules and resident needs.
