The Resident Care Coordinator plays a vital role in ensuring the highest quality of care for residents in assisted living and memory care settings. You will over see all Resident Assistants. This position requires a compassionate individual who is dedicated to enhancing the lives of seniors and individuals with disabilities. The ideal candidate will possess experience in caregiving, strong communication skills, and the ability to manage care plans effectively.
Responsibilities
Oversee daily operations of resident care, ensuring adherence to established care plans.
Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and meal preparation.
Provide support for individuals with developmental disabilities, Alzheimer's disease, dementia, and autism.
Administer medications as prescribed and monitor residents for any changes in condition.
Conduct regular assessments of resident needs and update care plans accordingly.
Maintain accurate documentation in EMR systems while adhering to HIPAA regulations.
Collaborate with healthcare professionals to coordinate patient care and address any concerns.
Scheduling resident assistants.
Train and supervise staff in caregiving techniques, first aid, and emergency response procedures.
Engage residents in social activities to promote mental well-being and community involvement.
Manage conflicts effectively and provide behavior management as necessary.
You are expected to manage and coordinate appropriate coverage when a team member calls off.
Skills
Experience in assisted living, nursing homes, or senior care environments.
Knowledge of medical terminology and patient care best practices.
Proficiency in direct support services for individuals with disabilities.
Familiarity with cleaning protocols to maintain a safe and hygienic environment.
Ability to perform heavy lifting when assisting residents with mobility needs (e.g., using a Hoyer lift).
Strong organizational skills for case management and documentation purposes.
Excellent communication skills for effective interaction with residents, families, and staff.
Understanding of vital signs monitoring and basic first aid techniques.
