Description/Summary

The purpose of this position is to provide leadership and direction to the View Unit staff as they provide care and service. This position will provide direction and coordination of the assessment and care planning process to ensure that each resident achieves or maintains the highest level of functioning.

Essential Duties and Responsibilities

ESSENSTIAL DUTIES AND RESPONSIBILITIES including the following:

  • Provide leadership to the View Unit team in a professional and positive manner, working closely with the interdisciplinary team to establish a positive working environment
  • Demonstrate conduct that is reflective of team collaboration and support. Communicate concerns and/or problems with the NHA and/or DON in a timely manner proposing possible solutions or remedies to the concerns and/or problems, be supportive of the final solution \xe2\x80\xa2 Manage the day-to-day functions of the unit activities in accordance with current regulations, best practice standards and Pleasant View guidelines.
  • Assist in developing methods for coordinating services to ensure continuity of residents\' care.
  • Complete assigned sections of the Minimum Data Set (MDS). Participate in the development of the care plan.
  • Provide reports for the facility\'s quality assurance and performance improvement program in a timely manner.
  • Participate in surveys (inspections) made by authorized government agencies. Conduct survey readiness audits, using the Critical Element Pathways.
  • Coordinate the admission, transfer, and discharge process with the resident and family in conjunction with the Clinical Assessment Nurse and other team members.
  • Attend various committee meetings of the facility, as required.
  • Attend Care Plan Conferences, ensuring that the person centered approach is maintained.
  • In collaboration with the Unit Nurse communicate with Medical Providers reviewing the plan of care with updates as to improvements, changes in condition and decline in condition.
  • Report and investigate all allegations of resident abuse, neglect, misappropriation of property, resident to resident altercation or injuries of unknown origin. As assigned will submit the online report to BQA. As assigned will complete the final report for submission to BQA.
  • Complete other duties as assigned.
Personnel Functions
  • Maintain professional appearance and professional presentation at all times.
  • Maintain office in an organized manner; free of clutter. Office is a secure environment that safeguards all confidential records, reports and information.
  • Actively prepare monthly goals and steps to achieve the goals in a collaborative manner.
  • Make daily rounds observing that team members are performing their work assignments in accordance with acceptable dementia care standards.
  • Review complaints and grievances made or filed by families and initiate appropriate steps to resolve the grievance.
  • Present disciplinary action in a private setting and administer the action in a professional manner. Establish a good understanding with the employee in regard to expectations.
  • Develop and maintain a good working rapport with personnel and other departments within the facility.
  • Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
  • Meet with the team on a regularly scheduled basis to promote effective communication, timely problem solving, and the improvement of services.
  • Attend all mandatory meetings and in-services and complete the make-up session in a timely manner.
  • Monitor that privacy and confidentiality of resident information is upheld. Monitor that facility computers are used for resident related services only. Monitor that employee cell phones are kept in the employee locker room or employee vehicle.
  • Review complaints and grievances in a timely manner. Monitor that the Grievance Policy is followed.
Other Functions
  • Develop and Manage the View Unit in accordance with \xe2\x80\x9cDementia Capable\xe2\x80\x9d criteria.
  • Oversee the routine of the day to ensure that it is filled with purpose for the residents and appropriate interactions.
  • In collaboration with the Nurse, review the Physician Review schedule and identify the updates that will be provided to the Physician. Participate as needed in updating the Physician.
  • Make daily resident visits to observe and evaluate the resident\'s physical and emotional status.
  • Monitor that appropriate behaviors are being tracked and that the care plan interventions are effective.
  • Provide ongoing education to team members and family members about dementia.
  • Update the DON and the Administrator of the status of residents as necessary.
  • Review the plan of care with residents, and/or family members, as necessary. Openly discuss concerns or challenges with the intent to find an agreeable plan. Care Functions
  • Inform personnel of new admissions and information about the new resident.
  • Audit the appearance and staging of the room so that it is ready when the resident arrives.
  • Greet the new resident upon admission.
  • Make daily resident visits to observe and evaluate the resident\'s physical and emotional status
.\xe2\x80\xa2 In collaboration with the Nurse, consult with the resident\'s physician in planning resident care, treatment, rehabilitation, etc., as necessary.
  • In collaboration with the Nurse, notify the resident\'s attending physician and HCPOA when there is a significant change in the resident\'s condition.
  • Inform family members of the death of resident as necessary.
Safety and Sanitation
  • Monitor that work areas are maintained in a clean and sanitary manner
  • Monitor that all resident rooms are maintained in a clean, safe, and sanitary manner
.\xe2\x80\xa2 Monitor that personnel participate in fire safety and disaster preparedness drills in a safe and professional manner

.\xe2\x80\xa2 Monitor that personnel follow infection control guidelines.
  • Monitor that personnel follow established hand washing guidelines
Care Plan Functions
  • Participate in the development of a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.
  • Insofar as practical, encourage the resident to participate in the development and review of his/her plan of care.
  • Review resident care plans for appropriate resident goals, problems, approaches, and revisions based on resident needs, preferences and wishes
  • Observe that personnel involved in providing care to the resident is aware of the care plan and that care plans are used in administering daily care to the resident.
  • Complete regular audits using the Critical Element Pathways.
Resident Rights
  • Maintains the confidentiality of all resident personal health information.
  • Monitor that residents are treated fairly, and with kindness, dignity, and respect
.\xe2\x80\xa2 Monitor that nursing care is provided in privacy and that nursing service personnel knock before entering the resident\'s room.
  • Monitor that nursing service personnel are knowledgeable of the residents\' rights.
Qualifications

QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

EDUCATION and/or EXPERIENCE
  • This position calls for a minimum of two (2) years\' experience in senior services.
  • A BSW is required and a MSW is preferred.

Pleasant View Nursing Home is a 24-hour facility which requires continuous care for residents, therefore employees must meet attendance standards by arriving to work on time, work all scheduled shifts and extra shifts that are agreed to work and have an overall excellent attendance record

.\xe2\x80\xa2 Ability to use design data and information such as residents\' medical charts, MDS, data collection sheets, Home Health reports, financial statements, progress notes, admission and discharge documents, authorization logs, Patient Care Standards manual, Physician\'s Desk Reference, Federal and State regulations, computer software operating, accounting methods and educational curricula.
  • Ability to communicate verbally and in writing with residents, PVNH personnel, physicians, hospital and clinic personnel, Human Service agencies, Home Health services, residents\' families/guardians.\xe2\x80\xa2 Required to consistently communicate effectively and professionally with Green County citizens, clientele, employees and officials
.MATHEMATICAL SKILLS
  • Ability to add, subtract, multiply and divide; calculate percentages, fractions, and decimals.
REASONING ABILITY
  • Ability to use functional reasoning development in performing activities within rational systems involving diversified work requiring exercise of judgment
  • Ability to apply situational reasoning ability by exercising judgment, decisiveness and creativity in situations involving the evaluation of information against sensory or judgmental criteria
.PHYSICAL DEMANDS
  • Ability to operate office equipment and machinery including computer keyboard/typewriter, telephone, photocopier, fax machine
.\xe2\x80\xa2 Ability to exert light physical effort in sedentary to light work, involving pushing residents in wheelchairs.

WORK ENVIRONMENT
  • In regard to environment factors work under moderately safe and comfortable conditions with potential factors such as odors, smoke, potential violence, noise, disease.\xe2\x80\xa2 Ability to work in an environment caring for individuals with dementia who may at times be resistive and combative.
