Role Overview

We're seeking a Licensed Practical Nurse (LPN) to join our remote clinical care team. In this role, you'll manage a dedicated panel of patients enrolled in Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and Behavioral Health Integration (BHI) programs. You'll deliver exceptional care through telehealth check-ins, patient education, and care coordination, all from home.

What You'll Do

Manage a panel of patients enrolled in CCM, RPM, and BHI programs Monitor patient-reported data and biometric readings via remote devices Conduct telehealth visits and check-ins to assess health status and reinforce care plans Provide patient education and support for chronic conditions and behavioral health needs Collaborate with RNs, providers, and multidisciplinary team members for coordinated care Accurately document all interactions in the EMR and other digital tools Uphold clinical protocols, quality standards, and patient confidentiality
What You'll Bring

Active, unrestricted LPN license (required) 1+ year experience in a clinical setting (primary care, internal medicine, or similar) Must have experience working with behavioral health patients Comfortable using telehealth platforms, EMRs, and remote patient monitoring tools Excellent communication skills and strong patient empathy Self-motivated, organized, and comfortable managing a remote caseload Must reside in LA

Prior experience with

Compensation & Benefits

Pay:

$24-$30/hour (DOE)
