Job Summary
The Rehabilitation Admission Liaison is responsible for completing clinical assessments and assessing potential candidates for Inpatient Rehabilitation, obtaining final approval by the Rehabilitation admitting physician, and educating stakeholders regarding inpatient rehabilitation outcomes and requirements for admission. Responsibilities:
Essential Functions

  • Early identification of potential patients through effective communication with hospital care management team, physicians,
other referring facilities, and through clinical rounds as assigned. * Coordinates the transition from the referring facility or home to the rehabilitation setting.
  • Thorough on-site or telephonic clinical assessment of patients, including chart review, clinical physical screening, and interaction
with the physicians, nurses, and therapists directly involved in the candidate's care. Through this process a determination of
medical stability and readiness for program participation is made. Family members or significant others may also be interviewed
to obtain accurate information as to premorbid functional status, personal interests, cultural considerations, and discharge
plans affecting the rehabilitation admission. * Establishes appropriate contacts with attending physicians/consultants, care management team, and other involved healthcare
professionals as necessary to ensure timely admissions. * Works as the liaison case manager with the internal and external care management team to provide a cost-effective plan for
each patient. * Provides patient and family with education regarding Patient Rights, benefits of program(s), and expectations of participation,
and arranges tour of program(s) as needed. * Obtains insurance authorization
  • Verifies level of care and intensity of services.
  • Maintains regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state and
local standards. * Maintains referral, admission, and daily census report.
  • Keeps supervisor informed of progress/problems.
  • Completes the admission process to include appropriate documentation of patient assessment regarding reason for admission
to an acute inpatient hospital rehabilitation unit in accordance with CMS regulations. * Maintains compliance with all Orlando Health policies and procedures.
  • Recommends alternate level of care for patients denied admission.
  • Serves as a resource to staff and other healthcare professionals.
  • Promotes physician and community awareness.
  • Maintains Data related to program.
Qualifications:
Education/Training
Bachelor's degree from an Accredited School of Physical or Occupational Therapy or Master's degree from an Accredited School of Speech Language Pathology.
Licensure/Certification
  • Maintains current license as a Physical, Occupational or Speech Language Pathologist in the State of Florida.
  • Maintains current BLS/Healthcare Provider certification.
Experience
  • Minimum of two (2) years of work experience within specific required degree
