Hilo Benioff Medical Center is seeking a dynamic and experienced Registered Professional Nurse IV, Care Manager to join our Patient Services team. This position is based in the Social Work Services Unit at Hilo Benioff Medical Center, an acute and long-term care facility on the island of Hawaii that serves as a Regional Center within the Hawaii Health Systems Corporation. The role provides professional case management and social services across the East Hawaii Region, including affiliated hospitals and care facilities. Working weekdays, weekends, holidays, and various shifts, this position is focused on delivering outcome-oriented patient care, optimizing resource use, coordinating clinical services, and supporting continuity of care. The ideal candidate will have a strong understanding of age-specific and developmental needs across the lifespan, from infants to geriatric patients.
Key Responsibilities:

• Coordinate and manage patient care plans through comprehensive assessments, interdisciplinary collaboration, discharge planning, and ensuring alignment with regulatory, insurance, and facility guidelines.
• Facilitate discharge planning by assessing patient needs, arranging post-discharge services (transportation, transfers, community resources), and ensuring continuity of care.
• Participate in quality improvement initiatives by identifying eligible patients, implementing care measures, and contributing to case management tools and departmental projects.
• Serve as a liaison and resource by guiding staff on care coordination, regulatory compliance, and best practices while maintaining patient confidentiality and professional communication.

EDUCATION, EXPERIENCE, CERTIFICATION & LICENSURE:
EDUCATION: Graduated from an accredited school of nursing. While not required, graduation from an accredited nursing program with a Bachelor's Degree from an accredited college or university is highly desirable.
EXPERIENCE: One and one-half (1 ) years of professional nursing work experience. Certified Professional in Utilization Review or Certified Case Management Certification preferred. Utilization management and case management experience preferred.
Supervisory Aptitude is the demonstration of aptitude or potential for the performance of supervisory duties through successful completion of regular or special assignments which involve some supervisory responsibilities or aspects; by serving as a group or team leader; previous work experience in a position that demonstrates or performs supervisory functions or in similar work in which opportunities for demonstrating supervisory or administrative capabilities exist; by completion of training courses in supervision accompanied by application of supervisory skills in work assignments; or by favorable appraisals by a supervisor indicating the possession of supervisory potential.
LICENSURE: License to practice as a registered nurse in the State of Hawaii.
CERTIFICATION: BLS certification is required within one (1) month of appointment.
