Job Description

General Statement of Duties:

The

Registered Nurse (RN)

provides nursing care to patients and families based on assessed needs reflected in the plan of care.


Level of Responsibility:

Reports to Nurse Manager, Cacapon team (Morgan and Hampshire counties of West Virginia)


Status:

Non-exempt, hourly, full time


Pay Range:

$36.95/hour - $43.20/hour; night shift differential $2 (Monday-Friday, 8 pm - 8 am); weekend shift differential $3 (Saturday and Sunday, any times); PRN (Occasional Part Time) status differential 5%; additional compensation may be available for additional education/certifications


Schedule:

Monday - Friday, 8 am - 4:30 pm; participates in RN weekend rotation


Service Area:

This position travels within the Cacapon team's service area including Morgan and Hampshire counties of West Virginia.


Duties and Responsibilities:

*Denotes essential job functions.


Assesses the physical, safety, and psychosocial needs of patients and families as they relate to nursing problems for developing and updating the plan of care. Provides direct nursing care as directed by the plan of care, respecting patient/family autonomy. Establishes and revises the nursing and hospice aide components of the plan of care with team collaboration and the consent of the patient and family, and updates plan as appropriate. Reports abuse and neglect to the appropriate authority following organization policy. Establishes and maintains good working relationships with the patient and family, the attending physician, coworkers and outside agencies. Assists the nursing facility nurses by contacting patient's physicians to update them on changes in condition, recommending and obtaining medication changes, and providing wound care. Supports and educates patient's families and acts as a resource for the nursing facility staff.


7. *Establishes and revises the plan of care with team collaboration and the consent of the patient and family and updates as appropriate.



8. Serves as a resource for the physician and nursing facility staff on pain control and symptom management.



9. Provides updates to physicians as required.



10. *Educates patients, families, and nursing facility staff regarding the Hospice of the Panhandle philosophy, stages of dying, progression of disease, and pain and symptom management.



11. *Documents assessment, nursing interventions and patient/family teaching according to guidelines reflecting the plan of care. Documents all other patient related information.



12. Registered Nurse assesses need for hospice aide services, develops aide plan of care and updates aides as needed. The nurse supervises aide care and documents the hospice aide supervision biweekly.



13. When working in a facility, complies with facility-specific requests for reporting, documentation, PPE use, and other regulatory and facility requirements.



14. *Functions as part of an inter-disciplinary team recognizing, reinforcing, and enhancing the role of all disciplines within the team. Communicates with team to provide continuity of care.



15. Arranges for crisis care when necessary.



16. *Participates in providing 24 hours per day, 7 days a week on call coverage to patient/families as needed. Participates in back-up call coverage as needed.


+ Provides phone triage, assesses patient needs, and makes visits as needed.
+ Available by beeper or phone for duration of shift. Communicates with answering service if out of reach.



17. Crisis Care - Provides direct nursing services to patients and families on a shift basis during a time of crisis when additional nursing care is required. Gives end of shift report to appropriate staff member.



18. *Maintains visit average standards established by the organization.



19. Participates in nursing staff meetings, in-services, and staff development to develop and maintain hospice expertise.



20. Assists with staffing in the Inpatient Facility as needed.



21. Demonstrates competency as a hospice nurse.





The following duties are specific to a Nursing Facility setting:



1. *Establishes and maintains a professional working relationship with the nursing facility staff recognizing that hospice staff provide expertise in end-of-life care, including support and education, that is available 24 hours a day. Provides information to nursing facility staff, patients, family members and visitors about hospice and the services we provide.



2. Complies with facility-specific requests for reporting, documentation, PPE, etc.....



3. Participates in the nursing facility care plan meetings to assure that the hospice care plan and nursing facility care plan accurately reflect one another.



4. *Assists nursing facility staff members in identifying patients who would benefit from hospice services.



5. Obtains verbal or telephone order from physician and documents in nursing facility chart and in progress note per individual nursing facility policy.



6. Reviews nursing facility documentation for orders and patient information.



7. Places a copy of completed visit documentation on the patient's facility chart within 7 days of visit.



8. *Observes and participates in weekly wound care and documents accordingly. Follows nursing facility wound care protocols and reviewing with wound care nurse.



9. Per individual nursing facility policy, administers medication.



10. *Demonstrates competency as hospice nurse.



11. Complies with the employee standards.



12. Other duties as assigned.

Education & Experience:




Ability to establish and maintain good working relationships with the patient and family, the attending physician, coworkers and outside agencies. Current West Virginia eNLC

Registered Nurse (RN) license

is

required

. Minimum of one year nursing experience, home health or medical surgical nursing in an inpatient setting preferred. Provides own transportation, and a license to drive with current liability insurance. Driving record meets Hospice's insurance company standards. Computer proficiency required.

Physical Qualifications:




Ability to transport self to office and patient care settings throughout the service area. Ability to walk from car to patient's room in their care setting and perform essential job functions. Ability to lift and move patients and equipment. Able to lift 50 pounds. Ability to communicate verbally and in writing in English language. Ability to hear normal conversation in person and via phone. Ability to read normal or condensed print. Ability to use a computer Ability to withstand odors.

Equipment Used

: Telephone, computer, office equipment, and medical equipment and supplies in order to provide patient care.
