Job Description


Provides competent, compassionate nursing care based on scientific principles and the use of nursing process and a primary nursing model.

1.1 Provides pre-operative consultation for ostomy patients for teaching and stoma marking.

1.2 Provides initial and ongoing post-operative consultation on new ostomy patients.

1.3 Discusses the plan of care with the nursing staff.

1.4 Provides wound and ostomy care across the continuum of care.

1.5 Provides consultative service for wound, fistula, percutaneous tubes/drains and ostomy needs to the multidisciplinary team.

1.6 Follows a collaborative approach to the delivery of care.

1.7 Identifies, facilitates, and evaluates outcomes of nursing care for an individual patient or group of patients.

1.8 Coordinates involvement of the patient, family and health team members in patient care, including patient/family teaching and discharge planning.

1.9 Documents relevant patient information using standardized Wound / Ostomy / Stoma Service templates.

1.10 Communicates the plan of care and other pertinent information to/from other health team members.

1.11 Helps define standards of excellence for patient care; participates in improvement of patient care services in a cost-effective way.

1.12 Introduces new equipment, products, nursing theory and research findings to improve specialized nursing practice.

1.13 Performs specific functions as described in the unit Scope of Service.

1.14 Provides care appropriate to age group of patients on assigned unit(s) as described in

relevant unit Scope of Service.

  • Participates in and contributes to performance improvement activities.
1.16 Supports the independent nursing practice of professional nurses through collaborative,

consultative and role modeling activities.

1.17 Participates in the Wound Task Force for the inpatient population with CNSs.

2.0 Develops, implements and evaluates educational programs based upon assesses learning needs of patients,

families and staff.

2.1 Identifies learning needs then develop and implements programs for patient education.

2.2 Collaborates with unit staff, CNSs and other health care professionals to meet staff and patients

Educational needs.

2.3 Facilitates the acquisition of advanced clinical skill in the staff nurses through role modeling, practice

and consultation in the clinical setting.

3.0 Provides expert consultation to patients, families, nurses and other health care providers to promote positive

patient outcomes.

3.1 Initiates, facilitates and develops interdisciplinary collaboration.

3.2 Provides consultation at the unit, program, service and community levels.

3.3 Acts as a resource to colleagues and patients.

4.0 Participates in and supports research activities that will positively affect patient care and patient care

outcomes.

4.1 Uses nursing theories and research findings to plan, design and evaluate nursing practice.

4.2 Participates in quality assurance and performance improvement activities for evaluation of

Structure, process and outcome criteria as it relates to clinical practice.

4.3 Participates in and supports clinical research.

5.0 Demonstrates accountability for ongoing personal / professional development.
  • Maintains and updates clinical knowledge and skills based on current nursing, wound / ostomy and
health care practices.
  • Identifies own learning needs and goals and designs a plan to meet them.
  • Participates in activities that foster peer support and networking.
  • Fulfills professional role through involvement in professional organizational activities, presentations
and / or publications

5.5 Within the framework of the nursing Scope of Service (1.32.02) and The Clinical Recognition Program (See guidelines, http://pcs.mgh.harvard.edu.), collaborates with nurse manager /clinical nurse specialist to identify his/her developmental level of practice (attached); with clinical leadership, identifies appropriate learning experiences to enhance development.
  • Adheres to Department of Nursing requirements for required training.
  • Contributes to relevant committees, professional meetings, continuing educational and/or formal academic programs.
Qualifications -
  • BSN required.
  • Current licensure as a registered nurse in the Commonwealth of Massachusetts.
  • Current certification in Wound / Ostomy Nursing.
