Responsible for gathering patient information, screening, and assessing pregnant patients to provide support in accessing appropriate prenatal care. Complete initial prenatal intake visit, including collecting records, personal health history and other screenings as needed. Identify high risk factors and follow appropriate protocols for continuity of care. Provide education around pregnancy, dietary guidelines, childbirth education options and exposures. Collaborate with OB Chair, Lead APPs, and Prenatal Care Coordinator when performing risk assessment of incoming transfer patients to determine imminent patient care needs, such as ultrasounds, labs, vaccinations, and referrals. As well as reviewing their records from previous facilities, update chart and enter any lab results into EPIC.
Graduate of an accredited nursing program. BSN preferred. 1-2 years of OB/GYN, L&D, or care coordination experience preferred.
NH RN license. BLS within 3 months of hire and maintained in accordance with quarterly RQI requirements. Active AWHONN membership encouraged.
