What you'll do (your impact):

Provide individualized, evidence-based nursing care in the client's home setting, following personalized Plans of Care. Administer and monitor specialty and infusion therapies (for example: PICCs, central lines, TPN/long-term antibiotic infusions) and coordinate with pharmacy/physician. Conduct comprehensive assessments, monitor changes in patient condition, proactively communicate with the Director of Nursing and interdisciplinary team. Serve as the clinical leadfor each assigned case: educate and support clients and families, coordinate with home health aides, and act as a trusted liaison. Document thoroughly via mobile platform, maintain quality/safety standard, and promote best practices.

What we're looking for:

Indiana RN license (active, unrestricted). Minimum 2 years of recent nursing experience; home health, infusion, or skilled nursing background strongly preferred. Current CPR certification, negative TB test or chest X-ray. Valid driver's license, reliable vehicle, auto insurance; mileage reimbursement provided. Ability to travel within Lake County (and occasional nearby counties) and handle reasonable client mobility (lifting up to 50 lbs, bending/standing). Self-starter mindset, excellent communication skills, clinical judgment, and a passion for client-centered care.
