Responsibilities:

Provide skilled nursing care to patients in their homes, including assessments, wound care, medication management, and education. Collaborate with other healthcare professionals to develop and implement patient care plans. Monitor patient progress and make adjustments to care plans as needed. Maintain accurate and timely documentation of patient care and interactions.

Qualifications:

Active RN license in New Mexico. Minimum of 3 years of nursing experience, home health experience preferred. Strong communication and interpersonal skills. Ability to work independently and manage time efficiently. Compassionate, patient-centered approach to care.
