For Local Candidates pay rate: $75 to $87/hr (If your permanent address is within 50 miles from the facility)

For Travelers/Stipend Candidates (If your permanent address is outside the 50 mile radius from the facility):
Blended Hourly W2 Pay Rate: $80.00
Blended Weekly Gross Pay Rate: $3,200.00
Minimum Hourly W2 Pay Rate: $48.85
Min. W2 Weekly Gross (Taxable): $1,954.00
Weekly Stipend (Non-Taxable): $1,246.00

OR

For Traveler 1099 Candidates:
Pay Rate on 1099 to the Candidate: $87.00
Weekly 1099 Traveler Pay Rate: $3,480.00

We are seeking an

RN Case Manager

to join our dedicated healthcare team. This position plays a critical role in the active management and coordination of patient care, collaborating closely with providers and interdisciplinary teams to ensure optimal healthcare outcomes for patients. The ideal candidate will be a skilled communicator, proactive problem-solver, and passionate about patient-centered care.

Key Responsibilities:



Patient Care Coordination:

Collaborate with medical providers and interdisciplinary teams to develop individualized care plans aimed at maximizing patient health outcomes. Serve as the primary clinical contact for patients, coordinating care and addressing needs in a timely and efficient manner. Track referrals to Surgery/Specialty Clinics, ensuring necessary information is provided to providers and maintaining communication on referral progress.

Clinical Duties:

Perform nursing duties in the clinic, including patient assessment, documentation, education, and administering medications and treatments. Ensure safety and infection control standards are maintained, completing annual competencies as required.

Team Collaboration:

Foster effective communication and teamwork across departments to improve patient care and clinic operations. Participate in departmental and clinic team improvement efforts, contributing to a positive and efficient work environment.

Problem Solving & Process Improvement:

Proactively identify and address obstacles to patient care, working to remove barriers and streamline processes. Lead or participate in special projects to improve health services and patient outcomes.

Qualifications:



Education:



Graduate of an accredited nursing program (Bachelor's degree preferred) High school diploma or GED

Experience:



Minimum

of

2 years

of

general nursing experience


Licensure & Certification:



A valid, current, full, and unrestricted RN license in the state of Alaska Basic Life Support (BLS) certification

Knowledge, Skills, and Abilities:



Strong knowledge of the nursing process and the ability to apply it in patient care. Excellent skills in patient assessment, data collection, and anticipatory care. Ability to work independently, multi-task, and prioritize in a fast-paced environment. Exceptional communication skills, both oral and written. Ability to work collaboratively within a multi-disciplinary team and provide guidance when appropriate. Problem-solving skills, critical thinking, and the ability to maintain positive working relationships across departments.

Pay: $93,000.00 - $117,000.00 per year
