Registered Nurse (RN - Care Coordinator)

$38.22-$45.63/hr., we honor based on years of relevant experience

Hours: Days

8:00am-4:30pm

with every

4th weekend

rotation

Job description:

As a Care Coordinator at Samaritan Bethany, you will:

Manage care of residents based on the resident-directed care plan Provide professional communication with residents' interdisciplinary team Comprehensively assess, report changes, and track ongoing resident clinical needs Supervises direct care staff by providing direction, completing performance reviews and disciplinary action Make change to direct care staff schedule to ensure safe resident care Able to work in a fast paced environment and handle changes in daily tasks well regarding the resident care and provider follow-up. Critically analyze and make effective change that is best suited to benefit the residents care and staff tasks

Requirements:

Must have a MN RN License

and be in good standing CPR Certification Prior long-term care experience preferred
