Essential Duties

1. The clinical nurse uses the nursing process to deliver high quality comprehensive and individualized care to members of Yale Health. 2. Collaborates with members of the multidisciplinary team to support the work of the team, unit activities, and the accomplishment of positive patient outcomes. 3. Delegates responsibilities to appropriate members of the care delivery team based upon an understanding of others\' roles, scope of practice, knowledge, and capabilities to ensure optimal utilization of personnel. 4. Contributes to a positive work environment by being responsive to the need for change. 5. Maintains a collaborative work relationship with staff members through respect and recognition of cultural and human diversity. 6. Successfully completes the competency based orientation within designated time frame. 7. Our state-of-the-art facility at 55 Lock Street, is where our members receive most of their care from our 150+ providers. It is a 144,000 square foot medical facility with over 90 exam rooms, an Acute Care Department, a 15-bed inpatient facility with two negative pressure rooms, a diagnostic imaging suite including MR and CT scan, x-ray and ultrasound and a full-service retail pharmacy.

Working under the direction of the Nurse Manager for Ob/Gyn, the Registered Nurse assumes oversight of the daily operations of the Clinical teams within the Ob/Gyn department. As a critical member of the care team, demonstrates outstanding clinical judgement and critical thinking skills to support organizational goals that provide superior outcomes and high quality efficiencies in the delivery of care. Patient and Family centered care is demonstrated in all areas of operations. Adheres to a safety culture, fosters staff engagement and service excellence to promote a positive work environment. Works in close collaboration with nursing, clinician and physician staff to ensure standards of care are consistently met. Practically applies comprehensive knowledge of nursing standards and principles as well as quality assurance and infection control theory in daily management.

The Primary Care Team Nurse assumes responsibility for triage, assessment, care-coordination, follow up and transitional care, clinical evaluation and clinical decision making for Ob/Gyn at Yale Health. Assumes responsibility for prioritization of care and disposition of patients from telephonic encounters as well as scheduled and unscheduled encounters. Compliance with nursing principles, medication safety and adherence to scope of practice is a fundamental expectation. The role utilizes the nursing process of assessment, planning, intervention and evaluation while caring for our patients maintaining confidentiality and dignity; provides safe, effective evidenced based care tailored to the individual needs of the patient and family. As a core member of the care delivery team, partners with the team to facilitate the coordination of patient care utilizing the principles of patient/family centered care. Ensures clinical policies and procedures are consistently followed, initiates new policy when appropriate. Role includes initiatives and services that support organizational and strategic goals. RN role serves to support clinicians and all members of the care team to prioritize and anticipate patient care needs. Fosters ongoing improvements of service and a culture that prioritizes safety. Role requires the ability to interface with key clinical partners, ability to make difficult decisions and facilitate resolution.

Required Licenses or Certifications

Licensure as a registered nurse in the State of Connecticut.

Physical Requirements

May require prolonged standing, walking, use of sight, hearing and touch, moving/lifting patients of varying weights. Requires keyboard use and ability to sit in front of a computer screen for long periods of time.

Preferred Education, Experience and Skills:

Three or more years of experience in maternal/child health and/or Labor and Delivery; proficiency in an EMR - Epic or similar.

Demonstrated clinical ability in Maternal/Child Health and or labor and delivery. Proficiency in fetal monitoring and assessment of the OB patient.
