Key Responsibilities

Patient Care & Clinical Duties
Provide direct and comprehensive nursing care to residents in accordance with physician orders and established facility protocols.
Perform daily assessments of residents' physical and mental health status; document and report changes in condition to the healthcare team promptly.
Administer medications and treatments accurately, adhering to proper protocols and maintaining precise documentation.
Assist residents with activities of daily living (ADLs) as needed, ensuring dignity, safety, and comfort at all times.
Monitor vital signs, fluid intake/output, and other health indicators; take appropriate action based on observations.
Maintain infection control and safety standards throughout all nursing activities.

Coordination & Communication
Collaborate with interdisciplinary team members--including physicians, therapists, social workers, and nursing assistants--to ensure comprehensive resident care plans.
Participate in care planning, evaluations, and updates to ensure residents' medical and emotional needs are met.
Communicate effectively with residents and their families regarding health status, treatment plans, and care expectations.
Ensure continuity of care during shift changes through accurate and thorough reporting.

Documentation & Compliance
Maintain complete, accurate, and timely nursing documentation in accordance with facility and regulatory requirements.
Follow all state and federal regulations, facility policies, and nursing standards of practice.
Participate in quality assurance initiatives, audits, and safety inspections.
Assist in preparing for regulatory surveys and ensure compliance with long-term care standards.

Leadership & Mentorship
Provide direction, support, and supervision to Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs).
Promote a positive work environment that encourages teamwork, accountability, and professional growth.
Participate in staff meetings, in-service training, and continuing education as required.

Qualifications & Requirements
Active and unrestricted Registered Nurse (RN) License
in the state of .
Minimum of 2 years of experience
in
Long Term Care (LTC)
or
Skilled Nursing Facility (SNF)
settings.
Strong knowledge of nursing standards, geriatric care, and long-term care regulations.
Proven ability to handle medical emergencies calmly and effectively.
Excellent communication, organizational, and interpersonal skills.
Compassionate and patient-focused approach to care.
Ability to multitask, prioritize workload, and adapt to changing resident needs.
Proficiency in electronic health record (EHR) systems preferred.

Experience in leadership or charge nurse roles within long-term care.
Certification in

Work Schedule & Environment
Flexible shifts:
7:00 AM - 3:30 PM
or
8:30 AM - 4:30 PM
(final schedule determined by Administrator).
