Home Health RN - Orange County

Assignment Type: 13-week contract with potential to extend

Schedule: Monday-Friday, 8:30 AM-5:00 PM + 1 weekend per month

Job Overview

Clover Health Services is seeking an experienced Home Health Registered Nurse to support patient care throughout Orange County. This role involves completing home visits, managing a caseload using the point system, and providing high-quality skilled nursing services.

Responsibilities

Conduct patient home visits (goal: 25 points per week)

Routine visits = 0.75 points

Admissions = 2.5 points

Provide skilled nursing care including wound care, IV therapy, and case management

Complete accurate and timely OASIS documentation

Participate in 1-2 on-call shifts as needed

Collaborate with the interdisciplinary team to support patient outcomes

Requirements

Active RN license (New York)

Certified Home Health Agency (CHHA) experience required

OASIS proficiency required

Strong background in case management, wound care, and IV procedures

Knowledge of Homecare Home Base (HCHB) strongly preferred

BLS certification required

Compensation

Travel Weekly Pay-$3631, Blended Hrly Pay-$90.78

Local Weekly Pay-$3409, Hrly Pay-$85.23

Facility-paid mileage: $0.50/mile (between patient visits only)

Orientation

2-7 days based on experience

Training provided for triaging and workflow systems
