Somerset Home Health Care is looking for qualified, experienced Home Health Care Nurses. Must be licensed in the state of Michigan. You will be able to accept or deny cases since it is a contingent position. Most of our patients are located in the downriver and metro Detroit area.

. As an RN, you will provide high-quality, patient-centered nursing services in a home health setting, helping patients manage their conditions and improve their overall well-being. Key Responsibilities

Provide skilled nursing care to patients in their homes, following the care plan established by the physician and in collaboration with the healthcare team. Perform comprehensive patient assessments and update care plans as necessary. Administer medications and treatments as prescribed, ensuring patient safety and comfort. Educate patients and caregivers on care procedures, medication management, and health maintenance. Collaborate with physicians, home health aides, and other healthcare providers to ensure optimal care coordination. Monitor patient progress and report any changes in condition to the physician and the care team. Complete and maintain accurate documentation of patient visits, progress notes, and care plan updates. Participate in Quality Assessment and Performance Improvement (QAPI) activities to continuously improve patient care.

Qualifications

Active Registered Nurse (RN) license in Ohio. BLS and CPR certification required. Minimum of one year of nursing experience preferred, especially in home health or clinical settings. Strong clinical judgment with the ability to work independently and collaboratively. Excellent communication and patient education skills to promote long-term recovery. Ability to travel to patient homes and work in various home environments.

Physical Requirements

Ability to perform physical tasks, including bending, lifting, and standing for extended periods. Comfortably lift and assist patients as required. Ability to travel to patient homes and work in diverse environments.

Work Environment

Work directly in patients' homes, traveling between visits. Exposure to different home settings and potential physical risks, with safety protocols followed.

Schedule & On-Call Responsibilities

Flexible scheduling options (Full-time / PRN available). May require weekend or evening availability based on patient needs. Timely communication of schedule changes with the Director of Nursing or Program Director is expected.

: Transition to full-time with additional responsibilities.

Clinical Director Option

: Assume office-based Clinical Director duties with pay ranging from $30-$40 per hour.
