3-4 days/wk, full-time (e/o Saturday)

Dialysis experience not required - we will train you for 12 weeks!

GENERAL PURPOSE OF THE JOB

Describe the primary purpose or scope of the job. What must be accomplished and why is it performed. The Intermediate Registered Nurse (RN)- ICHD is responsible for the delivery of all dialysis related nursing services for the End Stage Renal Disease patient in a chronic setting. The teammate knows, understands, and follows teammate guidelines, employment policies and policies and procedures. Problems are typically identified within the immediate job tasks.

ESSENTIAL DUTIES & RESPONSIBILITIES

ESSENTIAL FUNCTION % OF TIME

1. Supervises and performs direct patient care including but not limited to taking and monitoring vital signs, initiation and termination of dialysis, documentation, physical assessment of patients and patient treatments, taking and transcription of physician\xe2\x80\x99s orders, drawing and documentation of ordered laboratory work 30%

2. Provides dialysis therapy, including but not limited to formulating dialysis plan per physician orders, weight and rate of ultrafiltration, verifying safety measures are in place and complied with, administering medications or other appropriate treatments, discontinuing dialysis according to established procedure, documenting all activity appropriately 20%

3. Analyzes patient assessments, laboratory data and, when concerns are noted, brings issues to the attention of the physician. Notes signs of trauma, patients\xe2\x80\x99 general physical and mental condition, reviews dialysis orders, documents observations in patient medical records. Communicates all emergencies, patient life-threatening complications and related incidents to supervisor, Medical Director, Regional Operations Director, Facility or Group Facility Administrator 20%

4. Assesses patient and family learning readiness and provides appropriate information on renal disease, prescribed dialysis treatment, and medical regimen and its impact on the patient\xe2\x80\x99s state of health and well-being. Participates in the development and implementation of the patient\xe2\x80\x99s short- and long-term plan of care and monthly progress note 15%

5. Provides unsupervised patient care, coordinates day-to-day work of Patient Care Technicians, and is a resource for new RNs 10%

6. Other duties as assigned. 5%

EDUCATION AND EXPERIENCE

Associate\xe2\x80\x99s Degree

At least 12 months\' experience as an RN

QUALIFICATIONS

Nursing degree from accredited school of nursing

Preferred Proficient in all Resident RN duties

Basic computer skills and proficiency in MS Word and Outlook required; functional proficiency in all computer systems of DaVita within 90 days of hire

CERTIFICATES, LICENSES, AND REGISTRATIONS

Current RN license in state of practice

Current CPR certification

Completes training programs approved by the medical director and the governing body as required by CMS guidelines prior to operating the water treatment system
