Education Requirements:

Posses and maintain a current, active West Virginia license in good standing to
practice as a Registered Nurse (RN) and AMAP Certification with OHFLAC.

Pay Range:

Negotiable

Responsibilities include:

1. On an on-call basis for triage, PRN medication administration and monitoring of all

AMAP personnel authorized to administer medications.

2. Triage of medically related incident reports.

3. Supervision and ongoing training/oversight of AMAP personnel.

4. Review all medication errors.

5. Attend IDT meetings to develop or review a client IPP.

6. Complete face to face annual nursing assessments, PRN nursing assessment and self medication evaluations for

all clients receiving nursing services from PAIS, Inc.

7. Develop and maintain Nursing Plan of Care for all eligible clients.

8. Development of DD6 medically related training for all staff and complete medical client specific training when

necessary.

9. Complete required DD9 form prior to each client\xe2\x80\x99s annual IPP per policy.

10. Develop and maintain nursing plans of care at least monthly and as needed.

11. Review medically related incident reports and critical incidents and attend Internal Review Committee Meeting

once monthly.

12. Writing, monitoring and supervising all nursing related habilitation programs and completing monthly medical

status reviews.

13. Document all services provided through a progress note.

14. Coordinate medical services with Director of Medical Services.

15. Hiring, certification, and direct training of all AMAP personnel.

16. Quarterly direct supervision of all AMAP personnel per OHFLAC requirements.

17. Oversee AMAP schedules with Administrative AMAP to ensure medication pass coverage.

18. Maintain AMAP Documentation Notebook in each office.

19. Develop and monitor medically related Health and Safety Protocols.

20. Temporarily assume specific RN responsibilities for neighboring office in the event no RN coverage is available.

21. Other duties and responsibilities as assigned by Director of Medical Services and Clinical Director
