includes prevention and screenings; urgent care for minor illness and injuries; a CLIA-waived lab for testing including strep, flu, covid, urinary track tract infections and hematocrit; chronic disease management; vaccinations; routine physicals; and individual, family, and group behavioral health services. SMART Clinics are open in full alignment with school days and hours.

Executive Summary:
The SMART Student Health and Wellness Model has partnered with Central Falls School District. Designated as "School Officials" under FERPA, each SMART Clinic has a full-time team of approximately five clinical staff, led by a licensed clinical provider who diagnoses, treats, and prescribes for illness and injury at the school clinic.
The SMART RN supports the FNP and LCSW to perform a variety of nursing and administrative related tasks to promote continuity and high-quality care. They play a key role in the vitaling/triaging of students, coordinating with the Health and Wellness Coordinator and all SMART Team members in keeping with the SMART Whole Child, Whole School approach. The person in this position must also exemplify SMART's Core Values of working with a sense of urgency, entrepreneurial spirit, customer-service focus, and servant leadership, as well as always exercising the utmost discretion and diplomacy when interacting with stakeholders, including students/patients, parents, faculty, and staff.
The ideal candidate will demonstrate an enthusiasm for public health and a deep commitment to educating, informing, and advocating on behalf of students, staff, and parents regarding matters of health, wellness, injury/illness prevention, and medical care, all within a state-of-the-art clinical setting.
The SMART RN will report to a combination of the medical provider, Ginn Group Collaborative as the Managed Services Organization, Nurse Manager, and Chief Medical Officer.
Qualifications/Requirements
Active RN licensure, consistent with state and local credentialing requirements.
Current CPR certification.
Fully compliant with Rhode Island required vaccinations.
Completion of required trainings, including Child Abuse Mandated Reporter and HIPAA compliance.
Current RIDOH Vaccines for Children (VFC) certification, with proficiency in vaccine storage, monitoring, and administration.
Demonstrated knowledge of Rhode Island immunization requirements, school health regulations, and communicable disease reporting protocols.
Skilled in reviewing and interpreting external immunization records.
Familiarity with ambulatory care equipment, including point-of-care (POC) testing, emergency bags, and AED devices; proficient in phlebotomy.
Strong computer skills, including proficiency in Microsoft Office, Excel, and Electronic Health Records (EHRs).
Proven ability to deliver accurate, high-quality work courteously and efficiently in a fast-paced environment.
The SMART RN may need to meet all required criteria and hold the necessary credentials to qualify for the School Nurse Teacher (SNT) "Emergency Certification" in accordance with state standards.
Bachelor's Degree Preferred
Bilingual proficiency preferred.
Able to properly document in Electronic Health Record (EHR), currently NextGen.

Provide high-quality clinical services to students, families, and staff in alignment with SMART Clinic and Central Falls School District standards.
Demonstrate clinical competency in school nursing practice, including first aid, health assessments, and implementation of medically prescribed treatments and supports.
Support medical providers by carrying out clinical orders, including administering medications, performing on-site blood draws, rapid testing, immunizations, asthma treatments, and comprehensive health screenings.

Demonstrate commitment to the mission and vision of the Central Falls School District and SMART Clinic Model.
When applicable, fall SNT duties per statutes 216-RICR-20-10-4 and 216- RICR-40-05-3, including prescription management for students on 504s and IEPs, as well as maintaining school health records and mandatory compliance needs.
Review and maintain accurate, up-to-date medical records, ensuring completeness of medical histories, immunization status, and consent documentation.
Complete and maintain all charting and electronic records in EHR

and Skyward accurately and promptly. Maintain confidential health records for students in compliance with HIPAA privacy and data protection regulations.
Manage vaccine-related processes, including updating immunization data, completing RIDOH VFC documentation, maintaining vaccine inventories and logs, and when the clinic is not in operation, transporting the vaccinations for storage.
Communicate professionally and collaboratively with administrators, service providers, students, and families regarding student health concerns.
Engage in ongoing professional development to enhance clinical and educational practice.
Perform physical tasks associated with patient care, including lifting, transferring, stooping, and bending as needed.
Maintain working knowledge of applicable laws, nursing regulations, and professional practice standards.

Patient Experience, Health Information Management and Workflow Management
Provides efficient, high-quality services to patients who arrive for appointments, telehealth "SMART Co-Virtual Care" visits, or visit in person to request appointments or information on tests and procedures, bills and charges, referrals, and other matters.
Protected Health Information (PHI) Management: Maintains all PHI in accordance with HIPAA regulations, medical office standards, and best practices across clinics, EHR, and population health data platforms.
Workflow Management: Ensures optimal, efficient, and effective patient throughput by preparing examination rooms, updating patient records, and verifying that all EHR and population health data platforms are accurate and ready for the provider prior to each patient visit.
When applicable, provide seamless Integration of SNT Duties: Collaborates with non-clinical team members to ensure the SMART Clinic remains in compliance with all RIDE and RIDOH student health reporting requirements, including but not limited to vaccine compliance and reconciliation, as well as height, weight, vision, hearing, and dental screenings.
Optimized Use of Nursing Credentials and Licensure: Functions at the highest level of professional license and credentials, under the direction of the clinic provider(s), to uphold clinical best practices and to ensure compliance with all student health services, data collection, and reporting requirements.
