RN - Care Manager

Schedule:

Variable - 8:30a-4:30p or 4x10s (7:30a-5p); weekend & holiday rotation required

Contract:

13 weeks | 40 hrs/week

Weekly Pay:

$1,914/wk ($51.04/hr) - $2,123/wk ($56.61/hr)

Role Overview:

Seeking experienced

RN Care Managers

to coordinate patient care, manage utilization review, and facilitate safe discharge planning. The Care Manager acts as the liaison between patients, families, providers, and payors, ensuring efficient, high-quality transitions of care within the hospital system.

Requirements:

Active RN license (ME or Compact) 3-5 years of recent Med/Surg, Case Management, or Utilization Review experience Discharge Planning experience required BLS certification (AHA) CCM or ACM preferred
