Description
The Registered Nurse (RN) provides supervision to clients receiving services in their homes under various programs. The RN is responsible for conducting initial and ongoing assessments, developing and implementing individualized care plans, and ensuring high-quality, patient-centered care that promotes comfort, safety, and independence in the home environment.

Required Knowledge, Skills, Abilities, Education, or Experience
Qualifications:
Current, active Registered Nurse (RN) license in the state of Kansas
Minimum of 1-2 years of recent nursing experience, preferably in home health
Current CPR/BLS certification.
Valid driver's license, reliable transportation, and proof of auto insurance.
Strong communication, assessment, and organizational skills.
Compassionate, dependable, and able to work independently.
Key Responsibilities
Perform comprehensive nursing assessments in clients home at minimum every 60 days and assist in development of care plans according to the client's needs and program requirements.
Ensure continuity of care by coordinating case managers, family, and caregivers.
Supervise and provide clinical oversight ensuring compliance with care plans and agency standards.
Provide training to caregivers as needed
Documentation & Compliance
Maintain accurate, timely, and complete clinical documentation in the system in accordance with state, and agency requirements.
Participate in calls and meetings to provide adequate care
Patient & Family Support
Provide education and training to clients and caregivers as needed
Support families in understanding and following the plan of care to promote optimal health outcomes.
Advocate for client needs and serve as a liaison between the client, family, and healthcare team
Job Type: Part-time
Pay: $25.00 - $35.00 per hour
Expected hours: 15 - 20 per week
