Registered Nurse Case Manager
Job Description
The RN Case Manager is responsible for coordinating and managing the delivery of quality care through the development, implementation, and evaluation of individualized patient care plans. This role involves discharge planning, managing complex outpatient needs, and ensuring continuity of care across various healthcare settings.
The Case Manager conducts prospective, concurrent, and retrospective reviews of clinical information to determine medical necessity, appropriateness of services, and cost-effectiveness. Acting as an advocate for patients and their families, the RN Case Manager ensures that every patient receives the right level of care at the right time. In certain situations, the RN may complete in-home or hospital evaluations to assess patient status and needs.
Qualifications:
Associate Degree or higher in Nursing (required) Active and unrestricted state Registered Nurse (RN) license (required) Certification in Case Management (required, or ability to obtain within one year of hire) Minimum of two (2) years of experience as a Medical Case Manager (required) Current Basic Life Support (BLS) certification (required) Valid driver's license and ability to travel as needed Strong knowledge of healthcare systems and current medical standards of practice Excellent organizational, communication, and interpersonal skills Proficiency with Microsoft Office and electronic health record systems Flexibility to work evenings, weekends, and holidays as needed

Pay: $79,944.35 - $96,277.07 per year
