The RN Case Manager is responsible for coordinating and managing the delivery of quality care through the development, implementation, and evaluation of individualized patient care plans. This role involves discharge planning, managing complex outpatient needs, and ensuring continuity of care across various healthcare settings.
The Case Manager conducts prospective, concurrent, and retrospective reviews of clinical information to determine medical necessity, appropriateness of services, and cost-effectiveness. Acting as an advocate for patients and their families, the RN Case Manager ensures that every patient receives the right level of care at the right time. In certain situations, the RN may complete in-home or hospital evaluations to assess patient status and needs.
