We are searching for a reliable and compassionate Nurse to join our team as we build Empyrean Home Care from the beginning.
The Nursing Supervisor is responsible for the overall management and performance of the clinical operation of the company. This role assures that the organization is in compliance with state licensing requirements and all applicable state and federal regulatory standards are continually met.
Responsibilities:
Demonstrates working knowledge of state and federal regulations, conditions of participation, licensure laws, and accrediting body regulation, so that the organization is compliant with acceptable standards of practice
Ensures the development and maintenance of required Clinical policies of the organization
Demonstrates a working knowledge of clinical protocols/policies and procedures that use evidence based best practices to provide consistency and quality care performance outcomes
Collaborates with the clinical and operational team to develop and implement overall growth strategy for clinical operations and financial outcomes
Conducts functional assessments and supervises high intensity services
Monitors and/or participates in the implementation and deliver of the Individual Service Plan (ISP)
Supervises the delivery of services and ensures the strategies reflect the goals and objectives of the ISP
Monitors the progress toward achievement of goals in the ISP and makes recommendation for modification to the ISP, as appropriate
Performs comprehensive assessment and analysis of clinical processes regarding the delivery of clinical services, efficiency and effectiveness
Supervises and provides direction and support to the clinical staff (CNA's, HHA's, PCA's)
Serves as a resource to all agency personnel regarding clinical activities
Responsible for developing, updating, and renewing waiver participating individuals' clinical paperwork, and maintaining relationships with individuals
Participates in QA and Performance Improvement Programs
Participates in budget preparation and census projections for clinical services
Attends external meeting as needed and presents information on the organization to outside sources
Develops and maintains cooperative working relationships with other companies, professionals, and referral sources to encourage the exchange of information and services
Attend and participate in all state licensure/accreditation audits.
Qualifications
Bachelor's degree in Nursing from an accredited program
3+ years of experience in nursing, with at least 1 year in home or community health nursing required
Minimum of 1 year in a supervisory or management position
Current license as a professional, Registered Nurse
Certification in Nursing Administration, Community Health, or Home Care preferred
Valid driver's license and automobile insurance, per state guidelines
Must have use of automobile in good working condition
