Starting Pay: $77,820.50 Base salary with potential increases depending on education and experience.

Position Purpose: This position has responsibility for the oversight of medication administration for clients on the team, coordination with the psychiatric prescribers and medical providers of clients on the team. This position has responsibility to carry out rehabilitation and support functions and assist in treatment; medication education, support, and consultation to families; and crisis intervention in consultation with the multi-disciplinary team.

Main Duties and Responsibilities:
  • Administer medications, document response, maintain accurate medication lists, and report and document medication errors.
  • Assess client medical and nursing needs and make recommendations for the client\'s individualized service plan.
  • Provide education regarding mental illness, physical health concerns, chronic disease management, wellness, relapse prevention and medications to clients, families, and team members.
  • Coordinate and advocate with pharmacy to ensure accurate and timely delivery of medications.
  • Refer and connect Clients with medical, psychiatric, and other providers as warranted by client status. Assist clients to follow through on referrals.
  • Evaluate effectiveness of all psychiatric and medical services, and provide additional coordination, advocacy or intervention as needed.
  • Productive communication and collaboration with medical team members.
  • ACT Case Managers are responsible for working as a team, in concert with each client to develop an individualized plan of services and supports (Individual Treatment Plan) that will enable each client to live as independently in the community as possible. Services and supports might include activities such as assisting with skill development in the following areas: budgeting, housing, transportation, hygiene appointments, relationship building, symptom management, medication administration and obtaining or maintaining benefits.
  • To the extent requested by the client, provide collaboration and education to significant others and family members who are interested in assisting the client in their recovery process.
  • Assure that services are provided as developed by the client and team as documented in the Individual Treatment Plan.
  • In case of illness or client crisis, provide coverage for team members to assure and maintain continuity of care for each client served.
  • Assure that MHR productivity standards (i.e., Assertive Outreach, case load, face-to-face contact, contact frequency, on-call, shift leader, PM, etc.) are met, as outlined by the Team Leader & Clinical Supervisor.
Minimum Education and Experience:
  • Current Minnesota license as a Registered Nurse.
  • At least two years of experience in community based mental health work serving adults recovering from mental illness or combination of equivalent experience and training.
