Department Description: Want to feel the support of a caring team while having a sense of autonomy and flexibility? Build meaningful relationships with your patients and feel like your care is making a difference in their lives? Join the Adult Home Health team! Our Community Health Nurses deliver care to patients in the community, providing a wide range of skilled nursing care while working to the top of their licensure. We are a comprehensive team comprised of RNs, Physical Therapists, LPNs, LNAs, SLPs, Social Workers and administrative support, all collaborating together to support patients to remain living in the setting of their choice, their home.
Requirements:
Greater than one year of home or public health nursing experience preferred, or a combination of education and experience from which comparable knowledge and skills are acquired
Previous home care desired but not required. We are willing to train and develop a candidate who demonstrates a willingness and desire to learn home health nursing.
New Graduate Nurses are welcome to apply to our full-time positions!
*Our coverage area serves the Chittenden and Grand Isle counties and is segmented into four territories. Exact case load territory is assigned after orientation period and may not be the same as listed on this posting*
On-call: Not Required.
Incentives: $10,000 Sign On Bonus for full time external applicants, prorated for part time roles. Additional terms and conditions apply. Mileage reimbursed at Federal Gas Rates.
Requirements:

• Current RN licensure recognized by the State of Vermont required.
• Appropriate experience in specific clinical area. Varies by unit.
