We are seeking a highly motivated and compassionate Registered Dietitian Nutritionist to join our dynamic team at Carriage House Nursing and Rehab in Bay City, MI. As a key member of our healthcare team, you will play a vital role in providing exceptional nutritional care to our residents, ensuring their overall health and well-being.
Responsibilities:
Conduct comprehensive nutritional assessments and develop personalized meal plans for residents with various dietary needs and restrictions
Collaborate with healthcare providers to develop and implement effective treatment plans
Provide education and counseling to residents and their families on healthy eating habits, nutrition, and disease management
Maintain accurate and detailed records of resident nutrition care and progress
Participate in quality improvement initiatives to enhance the overall quality of care
Requirements:
Bachelor's degree in Nutrition or related field
Completion of an internship or residency program in dietetics
Registered Dietitian Nutritionist (RDN) certification
