A Day in the Life of a Registered Dietician Care Manager





The Registered Dietitian (RD) Care Manager assists patients and their caregivers to ensure safe and effective transfers across the continuum of care and educate patients to assist them in their healthcare decisions. The RD Care Manager serves as patient advocate, provides nutrition education and guidance for effective care transitions, and improved self-management skills.

Here is What You Will Be Doing:

Works collaboratively with physicians, hospital and clinical staff to track progress of patients and identify opportunities to assist in improving quality outcomes. Monitors patient's response to treatment, works with patient's team to analyze and address unanticipated developments, and assists in removing barriers to care. Manages individuals with focus on high risk populations across the continuum of care. Provides continuity of care management for those patients with multiple chronic conditions Builds trust and rapport with patient and/or primary caregiver, while assisting patient and/or primary caregiver to set goals for treatment and general health; provides post-discharge visits and phone follow up to patients who meet program criteria. Refers the patient to the appropriate community resources with the patient's understanding and cooperation. Acts as liaison with community and medical agencies to coordinate appropriate referrals for services. Documents all patient communications and coordination in the medical record as appropriate. Provides information and bring best practice to patient education, and support to patient's and/or primary caregiver regarding disease(s) self-management Provides point of contact for internal and external communication resources for patient and families.

Desired Skills:

Knowledge of clinical nutrition, specialized nutrition support, nutritional value of foods, interpretation and implementation of special diets, meal planning and preparation that is culturally appropriate. Ability to communicate effectively with patients, visitors, and staff Ability to work independently while collaborating with other team members Ability to provide evidence-based nutrition education to outpatients and their families Ability to organize workload and use effective time management Ability to drive to and from a variety of settings in varying weather conditions

Here is What You Need:

Current Wyoming Dietetics License Fourteen (14) Days: Cheyenne Regional AHA RQI within 14 calendar days of start date

Nice to Have:

Experience in motivational interviewing Experience in adult and pediatric weight management Experience in clinical nutrition in an outpatient setting
