Job Title: RN - Home Health

Rate: Up to $2200.00 weekly

Location: Clarksville, TN

Day (8H)

Key Responsibilities

Patient Care

: Administer medications, perform wound care, and monitor vital signs. Provide education to patients and families about health management and care procedures.

Assessment and Planning

: Conduct comprehensive assessments of patients' health conditions and develop individualized care plans in collaboration with healthcare teams.

Coordination

: Collaborate with physicians, physical therapists, and other healthcare professionals to ensure continuity of care and optimal patient outcomes.

Documentation

: Maintain accurate records of patient progress, changes in health status, and care provided, ensuring compliance with agency policies and regulations.

Support and Guidance

: Offer emotional support and guidance to patients and their families, helping them navigate their health challenges and treatment options.


Education

Graduate from an accredited school of nursing.

Experience

Prefer a minimum of one (1) year Adult Home Health experience

Licenses/Certifications

Have and maintain current licensure as a Licensed Registered Nurse within the state of employment. * Must be able to obtain and maintain CPR certification.
