Referral Coordinator


PRIMARY FUNCTION

The Referral Coordinator is responsible for scheduling all outside specialty and diagnostic appointments ordered by medical providers. The Referral Coordinator is a vital link between the primary care provider, the patient, and the specialist. The Referral Coordinator facilitates communication between all parties to ensure the appropriate information is exchanged and received.

PRINCIPLE DUTIES

Responsible for scheduling all outside specialty and diagnostic appointments, as ordered by the medical provider.

Receives and accepts tasks in inbox from the provider.

Obtains preauthorization from insurance company, if necessary.

Schedules specialty or diagnostic referral with outside practice or hospital within three business days.

Schedules STAT referrals within 24 hours of the time of the order.

Faxes order and necessary medical records to the facility receiving the referral.

Notifies patient of appointment date, time and location.

Documents referral appointment information including, location, date, time, and PHI sent in the referral template in NextGen.

Follows up with orders that are faxed to facility to obtain an appointment date.

Maintains referral and diagnostic log by contacting offices to identify whether patients kept appointments and requests records from appointment be sent to our office.
+ Ensures Diagnostic referral log is updated daily and records are received within one week.

+ Ensures Specialty referral log is updated weekly and records are received within four weeks.
Acknowledges that White House Clinics strive to be a patient-centered medical home and, as such, prescribe to illustrates the employee's role in aforementioned team-based care and accepts responsibility for being an active member of the team, including identification of quality improvement opportunities.

Performs related work and other duties as assigned.

Minimum Education


High school graduate or equivalent.

Minimum Work Experience


Experience in a health care facility, working with underserved patients.

Qualifications


Successful applicants will be mature and well organized with pleasant personality and speaking voice. A professional and friendly demeanor should be used when contacting patients and other medical offices in the community. Knowledge of medical office procedures, basic computer skills, and scheduling. Position requires skills in verbal and written communication and multi-tasking. Proficient time management and organizational skills. Preference will be given to candidate with previous medical office experience.

This is especially important in the areas of insurance coverage and claims, patient flow, scheduling, assuring correct patient information, and handling patient relations. Provide superior customer service to all patients, external customers (office staff of other offices, hospital, community agencies, etc.), and co-workers. Serves as an ambassador of the White House Clinics in all conversations and interactions with these parties.

Maintains clinic standards for a clean and quiet patient environment to maintain a positive patient care experience. Maintains compliance with organization's policies, as well as established practices, protocols, and procedures of the position, department, and applicable professional standards. Complies with organizational and regulatory policies for handling confidential patient information. Adheres to professional standards, clinic policies and procedures (OSHA, HIPAA, Standing Orders, etc.), federal, state, and local requirements, Health Resources and Services Administration (HRSA), U.S. Department of Health and Human Services (HHS), Office of the Inspector General (OIG), etc. Cooperates fully with all aspects of Corporate Compliance Plan and Standards of Conduct. Participates in all safety programs which may include assignment to an emergency response team.

TYPICAL PHYSICAL DEMANDS

This position requires sitting, some bending, stooping and stretching. Eye-hand coordination and manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator, and other office equipment are also required. Position requires normal range of hearing and eyesight to record, prepare, and communicate appropriate reports. Employees will be required lift papers or boxes up to 50 pounds occasionally.

TYPICAL WORKING CONDITIONS

Work is performed in office environment and involves frequent contact with staff and the public. Position may involve dealing with angry or upset people. Evening and/or weekend work is required. Work may be stressful at times.

Evaluated annually by the Team Leader or Service Line Manager with input from providers and support staff as appropriate.
