Job incumbent provides coordination of care for all clients participating in the Reentry Project. Incumbent will coordinate referrals for care and provide case management to clients until they are enrolled in appropriate services.

Representative Duties:
Under general supervision of the Reentry Clinical Lead, job incumbent will:
1. Provide individual and family screening and case management to clients, in coordination with the Directing Clinician. Case management encompasses a team approach and includes criminal justice supervising authorities, substance use and mental health treatment professionals as well as linkage to medical care to support long-term recovery.
2. Provide outreach services to clients, including routine phone and emergency outreach.
3. Network with area professionals to ensure follow-up care and referrals, including transfers of care among various Behavioral Health programs and social support services.
4. Provide coordination and support for assessments, using DSM 5 and ASAM criteria. Coordinate directly with other community agencies on inpatient and outpatient referrals for treatment.
5. In partnership with the provider team, complete comprehensive case management plans which directly address the risk factors for recidivism and the identified delivery of services.
6. Assist with urgent and routine requests for care, including community and provider requests, gathering pertinent history and records, and assisting with triage, in collaboration with the Direction Clinician, Psychiatrists and Medical Providers.
7. Provide prevention, psychoeducation, and support services to patients regarding mental health, substance use, recovery and overall wellness.
8. Provide and receive appropriate referrals to and from other agencies and resources, including but not limited to: Chief Andrew Isaac Health Center, Fairbanks Memorial Hospital, Anchorage Native Medical Center, Fairbanks community providers, external laboratories/testing services/imaging centers, inpatient and outpatient services, and correctional facilities, both in and out of state.
9. Assist with the provision of recovery support services that provide emotional and practical support to maintain client remission.
10. Coordinate the completion of regular drug testing for clients.
11. Assist with letters, written evaluations, care summaries and community resource agencies as needed, to provide additional client and provider support.
12. Complete documentation in a timely manner in accordance with guidelines established by Tanana Chiefs Conference, Indian Health Services, state and federal agencies and regulatory agencies.

Minimum Qualifications
Minimum Qualifications:
1. Bachelor's degree from an accredited university in biology, psychology, counseling, human development, or related medical or behavioral health field, or other Bachelor's Degree with appropriate work experience in these fields. Applicants without the educational requirements must have a minimum of four years' experience as a behavioral health case manager or coordinator.
2. Must have direct patient care experience, including work with children, adolescents or families.
3. One year work experience in primary care or behavioral health, addictions field, community case management or similar, is preferred. Must be familiar with mental health disorders, substance abuse and addiction, recovery and relapse prevention.
4. Experience working with Alaska Natives/American Indians preferred.
5. Must pass background check pursuant to the State of Alaska background check and federal Indian Child Protection and Family Violence Prevention Act requirements.
Knowledge, Skills & Abilities:
1. Cross-cultural treatment skills and knowledge of Alaska Native cultures and traditions preferred.
2. Strong computer skills using electronic health records and Microsoft Office applications.
3. Strong verbal communication, analytical and interpersonal skills required.
4. Must have initiative and prioritizing skills.
5. Must work as part of a team.
6. Must have excellent customer service skills and the ability to build rapport with clients, while respecting privacy and confidentiality.
7. Knowledge and ability to adhere to Health Insurance Portability and Accountability Act (HIPAA) and 42 CFR Part 2 confidentiality regulations.
8. Must demonstrate ethical commitment to mental wellness, recovery and goals of treatment within Tanana Chiefs Conference Health Services.

Physical demands:
Fairbanks area and village travel will be required to meet clinical need. May be exposed to certain conditions, conditions include but are not limited to: travel by small aircraft, vehicle, lack of running water, poor heat, sleeping on floors, and exposed to adverse weather conditions. Job incumbent must be able to physically travel by air and vehicle. Must be able to push, pull, lift and carry up to 40 pounds in support of equipment and supplies. Must be able to work in a busy, and sometimes stressful, clinic environment.
