PAY RANGE: $31.37 to $38.34 hourly
JOB SUMMARY/OVERVIEW:
Under the direction of the Quality Improvement (QI) Director, the QI Project Coordinator will work to maintain and improve the quality and performance of clinical services at CommuniCare+OLE according to the QI Plan through comprehensive and organized coordination of assigned Quality Improvement Projects.

MINIMUM POSITION REQUIREMENTS: EDUCATION, EXPERIENCE, SKILLS/TRAINING
Education:
High School Diploma required; prefer bachelor's degree or some college coursework, especially in a health-related field.
Experience/Lived Experience:
Minimum three (3) year administrative experience, and
Minimum three (3) year working in an Outpatient Primary Care Community Health Center setting or equivalent experience strongly preferred (required if no healthcare-specific education or degree)
Special Skills/Training:
Required: strong working knowledge of Microsoft Office Suite (including Excel), experience with electronic medical records systems (preferably OCHIN Epic)
Preferred: Knowledge of and experience with any or all the following: Quality Improvement Methodologies (IHI Model for Improvement, Lean, Six Sigma, etc.), Project Management training, Change Management
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
1.
Maintains a deep understanding of assigned projects (e.g. PCMH, Peer Review, PQI, Patient Satisfaction, Health Awareness Campaigns, Partnership Quality Incentive Program)
2.
Collaborates effectively with the QI Team to assure project goals are progressing as planned and necessary adjustments communicated and made as needed in a timely manner
3.
Assists the QI Project Manager with the peer review and patient satisfaction reporting processes by coordinating reviews, sharing service recovery opportunities, and supporting data analysis as needed
4.
Assists the QI Manager and Quality Improvement Coordinators in conducting panel management of patients and outreach to patients who are due for a variety of screenings, appointments and immunizations
5.
Assists with the Patient Centered Medical Home renewal process, including; documentation of workflows, ensuring collaboration with key stakeholders and assisting with advancing project goals and tasks
6.
Assists the QI Director and QI Manager with a variety of short-term projects including; awareness campaigns, outreach campaigns, data validation and data entry for program compliance
7.
Assists with drafting and disseminating updates with the organization on behalf of the department through emails, newsletters and other avenues, tailoring information to different audiences and groups
8.
Assists in preparing materials for meetings, including summaries, reports, and supporting documentation
9.
Assists the QI Project Manager with the PQI Process by tracking cases, updating data, and following up on pending requests to support timely review and closure
10.
Performs other duties as assigned.
