Job Description

Overview:

The Quality Improvement Coodinator provides nursing assistance and education as part of a multidisciplinary health care team to patients of varying socioeconomic and cultural backgrounds in an outpatient community health center setting. Responsible for care team achievement of clinic performance measure goals/performance metrics through leadership, delegation, problem identification and continuous process improvement.

Responsibilities:
Responsible to review and prioritize patient risk adjustment assessments as required by private insurers, managed care organizations. Responsible to promote and educate care teams to manage and achieve clinical performance metrics as set by CBHA. Responsible to review patient test results with provider and communicate with patients as needed to ensure effective outcomes. Responsible to engage in outreach events to promote and educate on preventive services. Responsible to review chronic care patient records and provide outreach services as required by Managed Care Organizations, patient insurers and medical providers on a monthly basis. Responsible to implement plans on assessed needs of patients with complex medical and social problems in coordination with multidisciplinary health care team. Responsible to provide nursing care including patient education assessments, nursing intervention, medical case management and procedures as needed. Responsible to communicate with equipment vendors to insure equipment is functioning according to manufacturer guidelines and safety protocols. Promotes a positive work culture and upholds clinic policies and procedures, including the WE CARE Standards of Behavior. Must uphold confidentiality at all times in regards to CBHA activities, reports, financials, patient health information and other proprietary information specific to CBHA. Other duties as assigned.

Qualifications:

Professional and Technical Knowledge:



Completion of a high school program and Medical Assistant training as recognized by the State of Washington. One year experience in a clinic setting. Ability to work under the full scope of practice that is outlined for a Medical Assistant - Certified under state of Washington regulations. 2+ years of experience in healthcare working with an electronic medical records system. Possess knowledge in proper lifting and moving of patients when assistance is needed. Knowledgeable and maintains compliance with recognized HIPAA regulations Maintains continuing education requirements of licensure.

License and Certification:



Current and unrestricted Washington Medical Assistant's license (MA-P, MA-R or MA-C).

Technical Skills:

Ability to prepare basic correspondence utilizing word processing and/or e-mail software. Ability to create tables and simple displays of information into spreadsheet software. Ability to enter and manage data into a centralized electronic health records. Ability to set up and coordinate phone conferences utilizing WebEx and conference room systems.

Communications Skills:



Possesses the ability to successfully form, foster and improve relationships with members of a culture different from one's own. Possesses ability to meet with a variety of people, both formally and informally, and to communicate in a manner that the receiving party will clearly understand. Job duties require the effective communication of information in written (including electronic) correspondence. Bilingual in English and Spanish to speak with patients.

In-house credit of $1,300 annual maximum for services provided at CBHA dental clinic. $35 copay is waived for preventative services.

Vision -

CBHA In-House Credit

In house credit includes for one eye exam every plan year. $200 hardware credit for employee and dependents for prescription hardware (glasses, contacts, prescription sunglasses). Employees and dependents also receive one additional hardware credit per year at CBHA's cost + 20% (used for non-prescription sunglasses, contact lenses, or a second pair of prescription glasses). $35 copay is waived for preventative services

403 (b) Retirement Plan:


Safe Harbor Match: Employees are eligible to receive a Safe Harbor Match of 100% up to 4% of the employee's total salary. Employee is full vested in Safe Harbor Match from day 1. Contributions are subject to IRS guidelines.
Non-Elective Contribution- Provided the employee has met enrollment qualifications, employee will also receive 4% employer contribution of employee's total salary at the end of the Plan Year at the discretion of Board of Directors.

Vacation (Personal Leave):

Year 1


40 hours front loaded after six (6) months of employment. Accrue at the rate of .0038per hour paid not to exceed a per pay period accrual of .3076 hours for the rest of year one (1). Vacation buyout paid hours are excluded from accrual

Year 2


Accrue at the rate of .0442 per hour paid not to exceed a per pay period accrual of 3.53hours. Vacation buyout paid hours are excluded from accrual.

Year 3


Accrue at the rate of .5576 per hour paid not to exceed a per pay period accrual of 4.46 hours. Vacation buyout paid hours

All employees receive 8 vacation hours for a Wellness Day on their Anniversary Date each year.
