Overview:
The Quality Improvement Coodinator provides nursing assistance and education as part of a multidisciplinary health care team to patients of varying socioeconomic and cultural backgrounds in an outpatient community health center setting. Responsible for care team achievement of clinic performance measure goals/performance metrics through leadership, delegation, problem identification and continuous process improvement.

Responsibilities:
Responsible to review and prioritize patient risk adjustment assessments as required by private insurers, managed care organizations.
Responsible to promote and educate care teams to manage and achieve clinical performance metrics as set by CBHA.
Responsible to review patient test results with provider and communicate with patients as needed to ensure effective outcomes.
Responsible to engage in outreach events to promote and educate on preventive services.
Responsible to review chronic care patient records and provide outreach services as required by Managed Care Organizations, patient insurers and medical providers on a monthly basis.
Responsible to implement plans on assessed needs of patients with complex medical and social problems in coordination with multidisciplinary health care team.
Responsible to provide nursing care including patient education assessments, nursing intervention, medical case management and procedures as needed.
Responsible to communicate with equipment vendors to insure equipment is functioning according to manufacturer guidelines and safety protocols.
Promotes a positive work culture and upholds clinic policies and procedures, including the WE CARE Standards of Behavior.
Must uphold confidentiality at all times in regards to CBHA activities, reports, financials, patient health information and other proprietary information specific to CBHA.
Other duties as assigned.
