is responsible for reviewing and ensuring the accuracy, completeness, and compliance of clinical documentation in accordance with state, federal, and accreditation standards. Working closely with the Director of Nursing, the QA Nurse helps maintain the highest level of patient care quality, regulatory compliance, and clinical integrity. Alora Home Health Software System

Key Responsibilities
Review all clinical documentation, including OASIS assessments, start of care, resumptions, recertifications, discharges, and visit notes for accuracy and compliance.
Verify all documentation and Plans of Care in
Alora
prior to billing or final submission.
Audit charts for compliance with
Medicare, Medicaid, CHAP/CMS
, and
state
regulations.
Provide timely feedback and correction requests to field clinicians regarding documentation errors or omissions.
Ensure accuracy and consistency of OASIS data and coding alignment with the Plan of Care.
Track, document, and report clinical deficiencies or trends to the Director of Nursing.
Participate in
QAPI (Quality Assurance and Performance Improvement)
initiatives and agency audits.
Monitor timeliness of visit documentation and follow up on outstanding records.
Support onboarding and training of new clinical staff on proper documentation and
Alora EMR
usage.
Maintain HIPAA compliance and confidentiality at all times.
Qualifications
Registered Nurse (RN)
licensed and in good standing in the State of [Insert State].
Minimum of
2 years of Home Health experience
required.
Prior experience reviewing OASIS and clinical documentation required.
Experience using Alora Home Health Software
preferred.
Thorough understanding of
Medicare Conditions of Participation (CoPs)
and state home health regulations.
Excellent communication, organizational, and analytical skills.
Ability to work independently, prioritize tasks, and meet deadlines.
Preferred Skills
Knowledge of CHAP or ACHC accreditation standards.
Familiarity with ICD-10 coding and care coordination documentation.
Proficiency with Microsoft Office applications (Word, Excel, Outlook).
Work Schedule & Expectations
PRN position:
Hours may vary based on agency needs and volume of clinical documentation.
May work remotely or in-office as assigned.
Must be available for occasional meetings or training sessions as needed.
Expected to complete assigned chart reviews within designated timeframes.
Performance Standards
Maintain a
95% or higher accuracy rate
in QA reviews.
Complete reviews and feedback within established turnaround times.
