Qualified Mental Health Professional (QMHP) - 1915(i) Services
Position Summary
The QMHP will provide direct, community-based mental health supports and case management to individuals eligible under 1915(i) HCBS programs. The role focuses on facilitating clients' community integration, independent living, recovery-oriented functioning, and coordinating services in alignment with person-centered planning.
The QMHP will ensure that services meet the policy and regulatory requirements for 1915(i) services and collaborate with multi-disciplinary teams, including psychiatrists, licensed clinicians, care coordinators, and support staff.
Key Responsibilities
Conduct initial and ongoing psychosocial assessments, including evaluation of needs, risks, strengths, and goals in the context of 1915(i) eligibility and services.
Develop, implement, monitor, and revise individualized service plans (ISP) or person-centered plans (PCP) in collaboration with the individual, family, and support team.
Provide direct service delivery (individual, group, family as appropriate) to enhance skills for community living, social integration, self-help, problem-solving, and recovery.
Support clients in achieving goals such as obtaining stable housing, employment, education, community engagement, and social supports.
Coordinate with other providers, payers, health plans, case managers, and community resources to ensure integrated service delivery and continuity of care.
Assist clients in navigating benefits, Medicaid eligibility, crisis prevention/intervention, and transitions between care settings (including from institutions to community).
Monitor service delivery for compliance with 1915(i) regulations (e.g., HCBS setting requirements, service documentation, billing standards).
Maintain detailed records, progress notes, service authorizations, outcome tracking, and data required for program evaluation and audits.
Provide training, consultation, and support to direct service staff and supervisors when applicable.
Ensure cultural competency, respect for individual preferences, and advocacy for client rights and community inclusion.
Minimum Qualifications
Bachelor's or Master's degree in a relevant human services, social work, psychology, counseling, rehabilitation, or mental health field (exact degree level may depend on state/county requirements).
Demonstrated experience (e.g., 1-2 years or more) working with individuals with serious mental illness, behavioral health needs, or in a community mental health setting. Eligibility for or holding any state-required credential for QMHP (or equivalent designation) in your jurisdiction.
Strong assessment, diagnostic, treatment planning, and case management skills; ability to develop measurable objectives and monitor progress.
Knowledge of Medicaid, home & community-based services regulations, 1915(i) service definitions, and HCBS standards (or willingness to learn).
Excellent communication, collaboration, time-management, documentation, and cultural competency skills.
Ability to work independently in community settings, with travel to client homes, neighborhoods, and service sites as required.
Preferred Qualifications
Experience in working with individuals with serious and persistent mental illness, co-occurring disorders (mental health + substance use), or dual diagnoses.
Familiarity with person-centered planning approaches, recovery-oriented care, supported employment, housing stabilization, and community integration.
Ability to supervise/direct junior staff, provide training, and lead team meetings.
Proficiency with electronic health records, Medicaid billing, outcome monitoring, and data-reporting.
Working Conditions & Physical Requirements
Work is performed in community settings (client homes, community centers, service sites) as well as office work.
May involve travel throughout the service region; valid driver's license and reliable transportation may be required.
Some work may occur outside normal business hours (evenings/weekends) depending on client needs or crisis responses.
The role may require carrying out crisis intervention, behavioral management, or safety planning in collaboration with other staff.
Supervision
The QMHP reports to a licensed clinical supervisor (e.g., Licensed Clinical Social Worker, Licensed Professional Counselor, Psychologist) and works collaboratively with the agency's clinical leadership. The QMHP may supervise or mentor paraprofessional staff or associates as specified by the agency.
