Location: 10 Corporate Hill, Little Rock and local schools




General Summary:

This position works under the direction of the Outpatient Director. Works with the Clinical Therapists to provide case management services for active Outpatient cases. This is a safety sensitive position.




Duties & Responsibilities:

Provides case management services for assigned families with mental health issues. Serves as a family advocate with other agencies that provide services, including but not limited to, juvenile court, schools, DCFS and others. Serves as a liaison with these providers, monitoring and ensuring patient and family compliance. Monitors and offers ongoing support, guidance and interventions to the families through telephone contacts. Drives and provides own transportation. Provides contact weekly in the natural environment of the child, per the treatment plan expectations. which includes the home, school or community. Provides documentation that the goals and objectives of the Master Treatment Plan are being met. Collects, coordinates and documents all information regarding families. During holiday, school, and summer breaks, this role will also help out in the QRTP program to help ensure proper ratios for that program.

Education and/or Experience



Bachelors degree required. Good driving skills, a valid Arkansas drivers license and eligibility for agency insurance required. Reliable transportation required. Paraprofessional certification is required within 30-90 days of employment with training provided on site. Past experience in mental health and utilization of community resources is preferred. Must be 21 years of age or older.




Language Skills



Must be able to communicate effectively both orally and in writing. Must have the ability to read, analyze and interpret professional journals, technical procedures and government regulations. Must have the ability to write reports and other communications. Ability to effectively present information and respond to questions from managers, patients, licensed associates and the general public.




Mathematical Skills



Must have a basic understanding of calculation of figures.




Reasoning Ability



Ability to define problems, collect data, establish facts and draw valid conclusions.




Physical Demands



Team members are required to sit, stand, talk, hear, see, walk and reach with hands and arms. Must also be able to personally restrain patients. Must maintain a current tuberculosis skin test.




Work Environment



While performing the duties of this job, the team member is occasionally exposed to outside weather conditions. The noise level in the work environment is usually moderate. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this job.
