Job Overview


We are seeking a dedicated and energetic QIDP/Program Coordinator to join our team in providing exceptional support and leadership within our community-based programs. This vital role involves coordinating services for individuals with developmental disabilities, ensuring personalized care plans are implemented effectively, and fostering a positive environment that promotes independence and dignity. The ideal candidate will possess a strong background in assisting with activities of daily living (ADLs), experience in Supported living or assisted living settings, and a passion for working with people with disabilities, including those with autism, dementia, or memory care needs. This position offers an opportunity to make a meaningful difference in the lives of residents while working in a dynamic, supportive environment.

Responsibilities



Develop, implement, and monitor individualized care plans tailored to each resident's needs, Coordinate and oversee direct support activities such as assisting with ADLs, medication administration, meal preparation, and vital signs monitoring Collaborate with interdisciplinary teams to ensure compliance with healthcare regulations including HIPAA Supervise staff and direct support staff , providing training on behavior management, conflict resolution, and safety procedures like first aid Conduct regular assessments of participants' physical and emotional well-being; manage behavioral issues through case management strategies Support residents' participation in community activities and outings that promote social engagement and independence

Qualifications



Prior experience working in assisted living facilities, nursing homes, or home care environments with individuals who have developmental disabilities or memory impairments Knowledge of Alzheimer's care, dementia management, autism spectrum disorders Proven ability to assist with ADLs such as bathing, dressing, grooming, mobility assistance and meal prep Familiarity with HIPAA compliance, and medication administration procedures Strong communication skills for collaborating with healthcare professionals, residents' families, and team members Valid driver's license for transportation duties; ability to perform light cleaning duties as needed Physical stamina for heavy lifting tasks; ability to work flexible hours including weekends or evenings if required

Pay: From $45,000.00 per year
