We are looking for a Public Health Program Coordinator to support participating health systems and their community partners involved in our client's program. This position will be full-time contract and hybrid.
What You'll Do
Coordinate and facilitate quality improvement work associated with the Primary Care Learning Collaborative and the Cardiovascular Learning Collaborative
Support and assist clinicians and medical staff within participating health systems on all aspects of health system improvements surrounding patient hypertension, cholesterol management, and other learning modalities
Facilitate team development within participating health systems
Provide coaching and facilitation using principles acquired through training activities and attendance at learning sessions
Share knowledge, skills, and examples of success from one health system with others
Assist health systems in integrating specific learning aims and change strategies with other related programs in the broader community
Foster partnerships between health systems, local health departments, and other community organizations
Provide on-site technical assistance and act as a liaison between participating health systems, technical support resources, and leadership
Work with health system CEOs, leadership, and external partners such as the Indiana Primary Healthcare Association
Provide technical support for participating health systems' staff involved in tracking improvements, as well as act as a liaison to technical support resources
Assist participating health systems in tracking indicator measures and quality improvement activities and submit them via an online reporting system
Coordinate measurement, analysis, and reporting of quality improvement progress, providing regular feedback to leadership
Build effective teams within health systems, incorporate quality improvement strategies, understand the use of data to manage populations of patients, improve care coordination for patients using multiple services, and improve efficiencies
What You'll Need
Required:
A degree in health science, health policy, administration, social science, or another relevant field
Knowledge of health systems and an understanding of chronic disease management models
Familiarity with the Chronic Disease Prevention and Management Framework, the Model for Improvement, and Collaborative Learning Methodology is an advantage
Excellent interpersonal awareness, verbal, written, and multi-media communication skills
Strong listening, problem-solving, group facilitation, conflict management, negotiation, and change-management skills
Ability to work effectively in interdisciplinary, multi-site teams
Proficient computer skills and knowledge of clinical information systems.
Self-motivation and enthusiasm
Physical Demands
Ability to safely and successfully perform the essential job functions consistent with the ADA and other federal, state, and local standards
Sedentary work that involves sitting or remaining stationary most of the time with occasional need to move around the office to attend meetings, etc.
Ability to conduct repetitive tasks on a computer, utilizing a mouse, keyboard, and monitor

$40 per hour
The pay range for this position is listed above. Exact compensation may vary based on several factors, including location, experience, and education. Benefit packages include medical, dental, and vision insurance, as well as 401k retirement account access. Employees in this role may also be entitled to paid sick leave and/or other paid time off as provided by applicable law.
