Job Requirement:

Common diagnosis / Types of patients: PT/OT - ortho, strokes, ortho, joints, spinal injuries speech
Additional Notes: Candidates must have experience with Oasis (gov't mandated form).
Must also have one year experience working PT/OT/Sp. Home setting experience is preferred but not required.

Job Summary:

The Physical Therapist will assess, diagnose, and create individualized treatment plans to help patients

restore function, reduce pain, and improve mobility. The ideal candidate will work closely with physicians,

nurses, and other healthcare professionals to provide high-quality patient care.

Key Responsibilities:

Evaluate patients' physical conditions and develop customized treatment plans. Administer therapy treatments, including exercises, manual therapy, and modalities. Educate patients and caregivers on rehabilitation techniques and injury prevention. Monitor patient progress and adjust treatment plans as needed. Maintain accurate and up-to-date documentation in compliance with healthcare regulations. Collaborate with interdisciplinary teams to ensure optimal patient outcomes. Adhere to all ethical and professional standards set by the American Physical Therapy Association

(APTA) and state licensing boards.

Qualifications:

Valid Physical Therapist license in [State]. Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program. Strong interpersonal and communication skills. Experience in outpatient, inpatient, or rehabilitation settings is a plus.
