Position Summary:

The Psychiatric Nurse Practitioner (PMHNP) will play a vital role in our mobile mental health team, delivering comprehensive psychiatric evaluations, diagnoses, and medication management in community-based settings such as clinics, senior living facilities, shelters, and other community spaces. This role is ideal for an independent and mission-driven provider committed to transforming mental health access and serving as an ambassador of Care2U's values in the community.

Responsibilities:

Conduct in-person psychiatric evaluations, follow-ups, and medication management in community-based settings.

Create and implement individualized treatment plans for patients across diverse populations.

Collaborate with primary care providers, therapists, facility staff, and community partners.

Perform crisis assessment and intervention when necessary.

Maintain accurate, timely documentation in the EHR system.

Comply with HIPAA and relevant state/federal guidelines.

Participate in case reviews, interdisciplinary team meetings, and continuous quality improvement.

Provide telehealth support for continuity of care as needed.

Travel locally within the service area; mileage reimbursement provided.

Qualifications:

Master's (MSN) or Doctorate (DNP) with board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC).

Active and unrestricted state NP and DEA licenses.

Valid driver's license and reliable transportation.

Experience in community or mobile healthcare strongly preferred.

Strong clinical and diagnostic skills with the ability to work autonomously.

Excellent communication and interpersonal skills.

Comfortable using mobile EHR systems and technology.
