Job

Purpose


The

Psychiatric APRN - Addiction Services

provides direct psychiatric care and medication management for individuals experiencing substance use disorders and co-occurring behavioral health conditions. In addition to serving as a clinical provider, this role contributes to the leadership and continuous improvement of addiction treatment services across the organization.


This includes:


Delivering evidence-based psychiatric evaluations, diagnosis, and treatment--including psychopharmacological interventions--for adolescents and adults with substance use and co-occurring disorders. Collaborating with the Chief Clinical Officer and interdisciplinary teams to integrate addiction services within Heartland's primary care and behavioral health model. Supporting the implementation of high-quality, patient-centered addiction care through consultation, care coordination, and occasional clinical supervision of integrated support staff (e.g., case managers, therapists, recovery coaches). Participating in the development and evaluation of protocols, workflows, and quality metrics to ensure regulatory compliance and clinical effectiveness. Serving as a subject matter expert and ambassador for addiction treatment in partnerships with community agencies and healthcare collaborators.
This role maintains an active patient panel and participates in collaborative care models to support whole-person, trauma-informed, and recovery-focused treatment.



Duties and Responsibilities


Program Development and Supervision:


Supervise and mentor addiction treatment staff, including counselors, case managers, and support personnel. Recruit, train, and support team members, fostering a culture of professional development and collaboration. Conduct regular staff meetings and performance evaluations to ensure high-quality service delivery Tracks time sheets, scheduling, and coordination of psychiatry team members for all leave types, to include but not limited to: PTO requests, attendance issues, and FMLA, in accordance with policies and procedures. Develop, implement, and evaluate addiction treatment programs, incorporating evidence-based practices. Identify opportunities for program expansion, including new services, locations, or populations. Collaborate with other departments to integrate addiction services into primary care, behavioral health, and other organizational programs.




Quality Assurance and Improvement:


Maintain and improve the quality and adequacy of medication-assisted treatment (MAT) services. Develop and update policies, procedures, and protocols related to medication-assisted treatment (MAT) practices. Monitor and ensure compliance with state and federal regulations, including licensure requirements and quality measures. Implement and oversee medication-assisted treatment (MAT) training programs to enhance clinical skills and service delivery.

APRN Services:


Oversee medication-assisted treatment (MAT) programs and ensure proper protocols are followed for SUD-related treatment and services. Act as a clinical resource for staff managing complex cases. Assess patients to determine if they meet the criteria for specific mental health disorders in an outpatient clinic. This role will conduct thorough evaluations, including medical history reviews, and psychological assessments, to reach an accurate diagnosis. Develop comprehensive treatment plans tailored to the individual's needs. This may involve prescribing medications, recommending psychotherapy or other evidence-based therapies, and providing guidance on lifestyle modifications. Evaluate patients by interviewing patients, family, and other people; conducting physical examinations; observing behaviors; reviewing medical history and related documents; selecting, administering, and interpreting psychological tests; and ordering laboratory tests and evaluating results. Conduct systematic assessments, diagnostic and functional evaluations of patients. Provide crisis intervention and assessment for patients seeking care or referred to by a healthcare professional. Prescribe and monitor medications used in the treatment of mental health conditions. They assess the patient's response to medication, adjust as necessary, and educate patients about potential side effects and interactions. Regularly assess the progress of their patients and adjust treatment plans as needed. Monitor the effectiveness of medications, evaluate the impact of therapy, and provide ongoing support to help individuals achieve their treatment goals. Collaborate with medical, dental, and other interdisciplinary teams to provide integrated care. Serve as a clinical resource for complex cases, supporting staff in delivering patient-centered solutions.



Qualifications


Master's or Doctoral degree in Nursing from an accredited program. Advanced practice specialization in the following area: Psychiatric-Mental Health Nurse Practitioner (PMHNP) Minimum of 5+ years of experience in an integrated health clinic with 2+ years in a leadership or supervisory role. Valid DEA Valid BLS Experience with medication-assisted treatment (MAT) and integrated care models preferred. Assessment, triage, diagnosis, planning, and implementation of a plan of care that is patient and family centered, developmentally and age appropriate, and culturally relevant Ability to collaborate, plan, develop, and evaluate evidence-based standards of care Ability to collect and analyze patient data to improve clinical outcomes Computer skills to navigate Electronic Health Record software; eCW experience preferred Federally Qualified Health Center or public health experience preferred

Working conditions


This role requires movement throughout the building day or multiple locations during the day. This role may be exposed to infections and contagious diseases. This role is subject to interruptions. This role requires management of personnel and procedures in a high stress environment. This role completes tasks that are performed in close physical proximity to other people. This role requires working indoors in environmentally controlled conditions.

Physical Requirements


Moves equipment weighing up to 50 pounds. Moves throughout the building to meet with employees, patients, or visitors. Must be able to remain in a stationary position during shift. Ability to read, write, and speak English. Frequent communication with patients about their experience at the health center. Must be able to exchange accurate information in these situations. Operate a computer and other office equipment, such as tablets, scanners, printers, and phones. Adhere to process protocol or procedures.

Schedule: Full-time exempt. + Hours Mon-Friday 8:00AM-7:00PM, as needed
+ Provider will average 34 patient facing hours per week with 6 administrative hours per week
+ Admin hours for this role will be required to be completed on-site
Location: All locations Heartland Community Health Center + Schedule may be altered by supervisor(s) as needed to allow for completion of expected duties of the position.
