Position Overview

Forefront Telecare is seeking an experienced Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) to join our remote Telehealth team. Forefront Telecare is seeking someone excited to collaborate actively in a team-based format to ensure coordination between our psychiatric and therapist providers. The position will entail working in collaboration with nurse practitioners, psychiatrists, therapists, primary care physicians, and clinical staff to provide quality behavioral and mental health services through our outpatient telehealth program. This position will allow you to work from home while serving patients accessing mental health care. Patient population is across the lifespan, commonly presenting with comorbid psychiatric and medical conditions, including depressive disorders, anxiety disorders, bipolar disorder, schizophrenia, substance use, and trauma/PTSD.

WE ARE LOOKING FOR:

Psychiatric Nurse Practitioner that is Licensed in Nebraska to do work via Telehealth for either of the following facilities:

Hospitals

  • Looking for NPs who have at least 4 days during the week (3-4 hours)
  • NPs on the weekend (3-4 hours on Saturday & Sunday)
  • Responsible for Daily Patient Rounding + On-Call work
  • Experience with Geriatric patients

What You\xe2\x80\x99ll Do/ Your responsibilities
  • Conduct psychiatric assessments/evaluations and diagnose clients according to DSM-5 or ICD-10 criteria
  • Formulate a working diagnosis, develop, and implement at treatment plan, and evaluate/modify therapeutic regimens to promote positive patient outcomes
  • When clinically indicated, perform assessment scales (e.g., PHQ-9, GAD-7, CSSR-S, etc.), order laboratory tests, review results, and document as required by Forefront Telecare guidelines
  • Prescribe, titrate, and monitor psychotropic medications
  • Participate in a collaborative approach of patient management using the knowledge and clinical resources of every member of the health care staff to provide the resolution of diagnostic and therapeutic problems and the implementation of effective solutions
  • Consult with members of the clinical team as appropriate
  • Provide individual and/or family education to increase each patient\'s awareness of illness and treatment issues
  • Participate in administrative or clinical meetings during scheduled hours
  • Perform activities and discussions related to coordination of care (PCP communication, calls with primary care practitioner, referral to higher or lower level of care etc.)
  • Organize care delivery in collaboration with the interdisciplinary team in a manner that promotes continuity and accountability for patient care
  • Promote team approach to patient care through creating and maintaining professional working relationships with physicians and support staff
Maintain appropriate medical records and complete all necessary clinical documentation in an accurate and timely manner in accordance with Forefront Telecare guidelines * Video (ZOOM) conference with patients, who have non-emergency medical needs, from your remote, secure location.
  • Evaluate and complete notes via EMR system in a timely manner.
  • Proficient in patient interview and mental status exams
  • Collection of comprehensive histories including diagnostic impressions
  • Pharmaceutical management
  • Safety evaluations
Requirements
  • Graduated from a state certified advance practice program specializing in a psychiatry/mental health
  • ANCC certification in Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC)
  • Must have active New Jersey state licensure as a Psychiatric Mental Health Nurse Practitioner (PMHNP)An unrestricted DEA registration in each state where direct services will be provided
  • A state-issued Controlled Substance Registration (CSR) in each state where direct services will be provided (if required by state)
  • Registration with the state Prescription Monitoring Program (PMP) in each state where direct services will be provided (if required by state)
  • No suspension/exclusion/debarment from participation in federal health care programs (e.g., Medicare, Medicaid, SCHIP)
  • Have an active license for the states which you are applying
  • Clean NPDB
  • Must be reliable with technology, have access to ethernet wired and/or Wi-Fi internet connection and strong computer skills
  • Multi-state licensed preferred
  • TelePsych experience preferred
  • Updated CAQH preferred
  • Computer navigation skills \xe2\x80\x93 Navigate EMR system (s)
  • Friendly and compassionate disposition
  • Effective communication skills
  • Team player
  • Knowledge of evidenced based interventions are required to care for patients of all ages who are experiencing different behavioral health crises and substance abuse.
Schedule
  • Weekdays (M-F)
  • Weekend available at certain facilities
