Job Description

The PRP Direct Care Worker is responsible for the direct provision of rehabilitation and support services to participating consumers. PRP Workers visit consumers in their homes and communities, and provide services in the home, community, or program site.

In this position you will:

Conduct regularly scheduled consumer visits.

Provide identified PRP rehabilitation and support services in accordance with the consumer's Individual Rehabilitation Plan (IRP).

Conduct Rehabilitation groups and activities to address rehabilitation and support service needs.

Maintain required and consistent documentation for each client, including contact/progress notes, monthly summaries and quarterly review.

Participate in ongoing training and supervision.

Understand the importance of self-determination in meeting a client.

Determine if the client is eligible or has Federal and State entitlements.

If necessary, assist with Federal and State entitlements.

Encourage community integration and local support services.

Other duties as assigned.

Engage in Professional Development

Provide in-depth post incident debriefs to leadership. High ethical standards with proven ability to handle highly confidential and sensitive information, excellent judgment, discretion, and diplomacy. Excellent written and verbal communication skills; ability to interface well with RBH leadership. Highly motivated, organized, and able to work effectively with minimal supervision. Proficient in Microsoft office.

Education & Requirements

Must have 2+ years as a direct care worker. Ability to speak, read and write English. Must possess analytical skills, detailed oriented and problem-solving skills. Must maintain a professional demeanor and perform duties effectively in stressful/emergency situations. Must be able to maintain objectivity and deal diplomatically with clients and family members. Ability to work independently and manage productivity. Must have CPR/First Aid Certificate.

Pay: $30,000.00 - $50,000.00 per year
