Under supervision of the Rehabilitation Specialist, the PRP Coordinator will be responsible for the coordination, provision, and continuity of services to individuals with a serious and persistent mental illness, to enhance self-management, community living skills and quality of life. Within a strength-based recovery model, the counselor provides support services to assist clients in achieving personal recovery goals and preferred lifestyles. The coordinator: a) evaluates for, plans, develops and implements functional and psychosocial interventions; b) assesses and coordinates internal and external services/resources; c) communicates with other care providers and primary supports as required.
Service provision is conducted individually and/or within recovery groups, and may occur at on-site or off-site locations
Duties
Teach consumers independent life skills and community integration skills
Provide supportive counseling to severely emotionally distressed consumers on caseload
Review and coordinate the collection of relevant information on referred consumers' background, social history and past treatment interventions to assist with the development of their Rehabilitation Plan
Implement, under the direction of the Rehabilitation Specialist, an Individual Rehabilitation Plan for assigned consumers to meet individual needs, interests, and abilities
Evaluate, monitor, and review individual treatment plans and assess individual progress in treatment, making recommendation for modification, changes or termination of treatment
Maintain all records as required by established legislation, policy, and administrative protocol and regulations including COMAR regulations
Attend all discharge, termination, admission and clinical supervision meetings as scheduled and applicable to promote consumer services or professional growth
May act as consultant to outside providers such as school personnel and medical staff, under the direction of the Program Director or CEO, in order to support the needs of the consumers on the caseload
Assist in identifying resources on behalf of the consumer, including medical, dental, mental health, housing, financial, food, clothing, and spiritual support resources
Responsible for assuring that consumer has the means to visit on-site at least once a month
Must be able to carry a caseload of 25 clients

Qualifications
High school or higher degree in Human Services related field or equivalent combinations of education and experience from which comparable knowledge and abilities have been acquired.
Have a valid Driver's License and an automobile;
Have 40 hours of Psychiatric Rehabilitation Program (PRP) training before independently providing PRP services.
Must have your own vehicle
Schedule
Monday-Friday 10am-7pm, some weekends may be needed
Reporting Authority
Must be supervised by the Licensed Rehabilitation Specialist and Program Director
