This is a professional position responsible for overseeing the development, tracking, and facilitation of projects related to transitions of care for Justice Involved-Re-entry (JI-R) populations as they move from a correctional to a community setting.
REQUIREMENTS
EDUCATION AND/OR EXPERIENCE:
Bachelor's degree in Business, Administration, Health/Science or related field and one year of project management experience; OR Associate's degree in Business, Administration, Health/Science or related field and three years of project management experience; OR high school diploma or equivalent and five years of project management experience.
Experience with population health data is preferred.
Advanced skills and proficiency in computer data entry and operation, including word processing, spreadsheets and databases. Experience with desktop publishing strongly preferred.
