The Population Health Program Manager is responsible for specific population health initiatives in the Appalachian Highlands region. This role involves community interaction, program implementation, and operational oversight, aimed at improving the health outcomes of the communities we serve. Key responsibilities include engaging with community members, local organizations, and stakeholders to understand health needs, promote health education, and foster partnerships that support health improvement initiatives. The Population Health Program Manager will also lead planning, development, and implementation of strategic initiatives to address specific health issues to support Ballad Health's goal of being a Health Improvement organization. They will be responsible for overseeing day-to-day operations of the population health program(s) they manage, ensuring they are running efficiently and effectively. This includes managing budgets, resources, and staff, as well as monitoring and evaluating program performance. The Population Health Program Manager will also work closely with healthcare providers, public health agencies, and community leaders to integrate population health initiatives with the goal of positively impacting community health. They will also ensure the program(s) they manage comply with all federal and state regulations. The program manager will prepare and present reports on program progress and outcomes and complete any grant reporting and budgeting as needed.
