A day in the life of a Program Manager can look like:



Assist the persons served in identifying and pursuing dreams while training in the skills that address their needs and wants as documented in the Individual Support plan. Implement team approved goals upon acceptance to program. After 30 days hold a meeting to review goals and revise as needed. Responsible to make contact with each new individual starting services on your caseload within 72 hours (unless otherwise approved by supervisor.) Identify needs and goals with the individual seven days prior to the thirty day staffing. Maintain all individual case files and electronic records for assigned caseload as it relates to licensing standards and PI policies and procedures. Review assigned case records according to quarterly and quality review procedures. Records will be maintained in an orderly and secure manner. Monitor goal progress and make changes as agreed upon by the full interdisciplinary team. Provide staff training for each individual's goals, supports and safety plans to ensure consistency and overall quality of services including training at the site. Demonstrate and assist with the use of redirection, de-escalating techniques to maintain a calm and flexible demeanor when confronted with disruptive behaviors. Carefully review daily logs of assigned caseload. Give timely and written feedback about quality concerns to the manager and coordinator. Provide training to staff as needed to improve logs. Communicate with supervisor on a regular basis pertaining to the overall services for the persons served on their caseload, including successes as well as concern areas. Keep informed of all programming and administrative changes by maintaining open communication with all team members, checking mailbox, and company's informational systems, if applicable, each shift. Responsible to prepare all other paperwork, as necessary, accurately and timely. Participate in meetings and in-services, as assigned. Assist person served and guardian in the application and resolution of benefits, financial matters (e.g. payee), burial contracts, social security, and legal matters. (Applies only to residential and only if P.I. is the payee.) Go to person served home (For percentage of time required) and make contact with person served and staff in 24 hour settings and person served apartments if applicable. Attend psychiatric/mental health appointments including therapy appointment, intake appointments with new providers i.e.: medical, employment, therapy etc., appointment related to any major health issues and appointments related to any change in condition.

NON-ESSENTIAL RESPONSIBILITIES:



Perform other related duties as assigned by your Supervisor.

MINIMUM REQUIREMENTS:



Be at least 21 years of age. One of the following qualifications must be met: a) A bachelor's degree from an accredited college or university in the social or behavioral sciences and one year post-degree experience in the delivery, planning, coordination or administration of human services, b) A high school degree (or its equivalent) and five years post-degree experience in the areas outlines in #1, and c) A combination of post high school education or experience as outlined in #1 totaling five years. At least one year must be experience. Maintain a valid Iowa Drivers License and a private automobile used to carry out job duties, as required by policy. Hold adequate liability insurance as required by policy. Attend all mandatory training, required by this position. Ability to teach and train all the skills necessary for persons served to pursue the most independent level of living their potential will grant them. Responsible to work business hours including a flexible schedule to meet the needs of Individual's on your caseload. Any schedule adjustments outside business hours would require approval by supervisor. Maintain a professional working relationship and effective communication with internal and external customers. Be proficient in computer skills to complete job duties as assigned, which may include Word, Excel, Therap, Person Served Financial Database, etc.

OTHER ESSENTIAL REQUIREMENTS:



Sociological

- must be able to communicate both in written and verbal form with persons served, parents, co-workers and other outside agencies. Must be able to work effectively and amicably with persons served, families, co-workers, people in the community and other departments within the agency.

Hearing

- must be able to communicate with parents, persons served, coworkers, outside agencies in person and on the telephone.

Sight

- must be able to fill out paper work and use a computer terminal. Must be able to decipher handwritten or typed material

Physical

- must be able to drive an automobile and have the physical stamina and dexterity as required for interventions. Be able to bend, lift 50 lbs, and ambulate stairs.

Job Type: Full-time

Pay: $20.27 per hour

Expected hours: 40 per week
