The Program Manager will lead strategy, operations, and staff oversight for the Brownsville Wellness Program.. The Program Manager will oversee the delivery of reimbursable services, supervise the CHW and RDN, build community partnerships, ensure compliance with SCN contractual requirements, and grow the program's reach to address chronic disease, food insecurity, and mental wellness in Brownsville.
This role requires strategic planning, contract administration, people management, and innovation to drive community impact and long-term sustainability.
Core Responsibilities:
Program Leadership & Compliance
? Oversee execution of SCN deliverables including:
? HRSN Screenings ? Social Care Plan creation and documentation ? Referrals and care navigation ? Nutrition education and MTM tracking
? Ensure timely billing and compliance via the SCN IT Platform.
? Maintain HIPAA-compliant workflows for all program operations.
? Develop internal systems for service tracking, impact reporting, and claims reconciliation.
Team Management
? Directly supervise and coach the Community Health Worker (CHW) and Registered Dietitian (RDN).
? Conduct performance reviews and support staff development, onboarding, and accountability.
? Lead internal team meetings and promote a culture of open communication and shared learning.
Partnership & Community Engagement
? Cultivate strategic partnerships with health providers, CBOs, churches, housing developments, and schools.
? Serve as the public face of BWP at coalitions, health fairs, city agency meetings, and advocacy events.
? Organize and lead quarterly community roundtables and wellness events. 4. Program Development & Expansion
? Lead rebranding of the Diabetes Wellness Program into the Brownsville Wellness Program.
? Oversee development of new initiatives in maternal wellness, mental health, and youth nutrition.
? Collaborate with the RDN to launch fee-for-service wellness programs, including corporate events and private consults.
Fundraising, Reporting & Sustainability
? Write grant proposals and steward existing funder relationships.
? Develop and monitor the program budget, working closely with finance staff.
? Submit all required reports to SCN, funders, and the Senior Director of Strategy and Programs, including: ? Monthly service reports ? Quarterly deliverables summaries ? Annual community impact report
? Identify and pursue Medicaid-aligned revenue opportunities to ensure long-term viability.
Skills & Qualifications
? Strong knowledge of Medicaid, social determinants of health (SDOH), and community-based wellness models.
? Exceptional leadership, supervision, and team development skills.
? Detail-oriented and organized, with experience in program evaluation and contract management.
? Proficiency in Microsoft Office Suite, Google Workspace, and data tools (Excel, Airtable, etc.).
? Strong written and verbal communication skills; comfortable presenting to funders and stakeholders.
? Culturally responsive and trauma-informed leadership approach.
? Knowledge of Brownsville/East Brooklyn communities preferred.
Education & Experience
? Master's degree in Public Health, Social Work, Public Administration, or related field strongly preferred.
? At least 2-3 years of experience in community health, nonprofit program management, or care coordination.
? Experience with Medicaid billing, health IT platforms, or SCN work preferred.
? Experience with grant writing and reporting strongly preferred.
