Summary:

Under the general supervision of the Division Director of Care Coordination, you will develop, implement, and manage day to day operations of WHS's Care Coordination Programs. Our Care Coordination services include assessment, service planning, linkage/referral, monitoring and follow up activities, transportation, and reporting services.




Education and/or Experience:



Bachelor's degree or non-licensed master level clinician required, with either degree being in behavioral health or human services preferred (psychology, social work, marriage and family therapy, counseling). A minimum 3 years' leadership experience required or experience in which supervisory abilities have been demonstrated. A minimum of 5 years' experience working in a human services setting. Must pass Criminal Investigation Background and DHHR background checks as well as post offer drug screen. Must have valid driver's license and current auto insurance.



Essential Duties and Responsibilities:

Oversee and manage a team of care coordinators and peers. Including providing guidance, support, and mentorship for staff in decision making and complex situations with clients. Organize and facilitate in-services/educational opportunities for staff, routine staff meetings, and group supervision. Oversee administrative responsibilities including, policies & procedures, compliance standards, HIPAA, incident reports, staff performance reviews, staff performance improvement plans, assist with hiring, assist with new employee orientation, payroll, etc. Oversee the coordination of services and contracts with community partners and/or funding sources. This includes reviewing referrals, reviewing documentation, meeting with supervisors and coordinators, and maintaining budget goals throughout the Fiscal Year. Oversee fiscal management by monitoring and approving staff billing for contracted services. Ensure staff are meeting productivity standards monthly. Represent Westbrook Health Service in the community and assist with networking and maintaining relationships with collateral contacts. Collaborate with other key staff within company, including Division Director of Outpatient Services, Outpatient Clinic Director, Executive Team, etc. Responsible for onboarding all new clinical staff to an appropriate comfort level with the agency EMR/My Evolv.

Demonstrate a willingness to learn and develop new skills to support the Care Coordination team and the clients we service. Including staying updated on current trend, resources, and educational opportunities in the MH/SUD and care coordination arena.

Excellent interpersonal and communication skills, ability to deal with changing priorities daily, flexible for all aspects of the job, highly dependable. Ability to coordinate, analyze, observe, make decisions, and meet deadlines in a detail-oriented manner. Proficient technology skills and knowledge of Microsoft Suite products, iPhones, and TEAMs. Ability to function with minimal supervision required. Knowledge of state agencies and community partners
